This is a synopsis to provide cost-per-test for reagents, consumables and preventitive maintenance in accordance with the Region 8 cost-per-test agreement on the Vitros 250, 950 and two Chemistry Analyzers. Contract is for USAF Academy Hospital and Peterson AFB Clinic. This solicitation is for commercial items prepared in accordance with the format in Federal Acquisition Regulations document (FAR) Part 12 and Part 13, as supplemented with additional information included in this notice. Solicitation number F05611-00-Q0069 is being issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 97-13. This solicitation has been approved as a Sole -Source Requirement for Johnson and Johnson Clinical Diagnostics 425 Hoes Lane, Piscataway, NJ 08855. However, all interested parties may provide a quote with documentation to support the ability to provide maintenance on the above referenced items.
