Department of Veterans Affairs, VA Boston Healthcare System seeks a
firm to provide organizational assessment and recommendation services
regarding Joint Commission Accreditation of Healthcare Organizations
accreditation evaluation, education and design services for the Office
of Safety and Occupational Health Management at the VA Medical Center,
Boston, Massachusetts. The Contractor is expected to conduct an
evaluation in accordance with the most recent standards of JCAHO, which
is a national organization dedicated to improving the care, safety, and
treatment of patients in health care facilities. The Contractor shall
provide Environment of Care consolidation and integration support and
strategy necessary to assist the VA Boston Healthcare System in
preparation for it's forthcoming Fiscal Year 2000 JCAHO Survey. The
contractor shall conduct or produce, at a minimum, documentation
reviews; evaluation of environment of care; facility hazard
surveillance; presentations; opening and closing conferences; a
recommendation report; workshops: an environment of care manual and;
JCAHO Environment of Care implementation and regulatory support.
Contractor shall work with VA staff to assist in implementing the
consolidation and integration program. At a minimum, the contractor
shall attend management meetings, provide program development, staff
education and technical support. A best value determination will be
based on the following the following factors in order of importance and
will be used to evaluate offers: (1) Technical capabilities (2) Past
Performance and (3) Price. Evaluation of your proposal will be
conducted as follows: Technical -50 points, Past Performance -- 30
points and Pricing -- 20 points. Under technical shall be subfactors
for Knowledge and experience with JCAHO requirements and regulations
(25 points) and quality of personnel (25 points). Your technical
proposal must contain a write-up of your proposed approach to the
services required including your proposed delivery schedule and resumes
for proposed personnel. The technical proposal is limited to no more
than ten pages, single spaced, 12 pitch. Soliciting past performance
information shall be done by the government. Your business proposal
must include a page labeled "Past Performance Contacts" which lists
three references for similar projects done within the past three years
with the name of the contact person who will be completing the
questionnaire the government sends, the contact's phone number, and fax
number with a brief description of the project performed for each
contact. The business/pricing proposal must include the following three
elements: (1) Labor -- the hourly rate for each labor category proposed
and the number of hours proposed; (2) other anticipated direct costs.
The hourly rate given must be supported by a detailed breakdown
indicating how it was derived, or, if the hourly rate is a previously
established commercial or government rate. The VA Boston Healthcare
System intends to award a firm-fixed price contract. Contract award is
Subject to the Availability of Funds. This procurement is a total
small business set-aside and the applicable SIC Code is 8742 and all
responsible offerors may submit an offer which will be considered.
