This is a combined synopsis/solicitation for commercial items prepared
in accordance with the format in Subpart 12.6, as supplemented with
additional information included in this notice. This announcement
constitutes the only solicitation; proposals are being requested and a
written solicitation will not be issued. The solicitation number is
F64133-99-R0023, and is issued as a Request for Proposal (RFP).
Contractors are asked to provide a lump sum proposal to provide all
parts, labor, material, equipment and transportation to repair cracks,
resurface and paint the base swimming pool, Building (bldg) 1620,
Andersen AFB, Guam. Performance period for this solicitation shall be
60 days from contract award date. This solicitation document and
incorporated provisions and clauses are those in effect as of Federal
Acquisition Circular 97-12. This requirement is a total Small Business
set aside. The Standard Industrial Classification (SIC) Code is 8999,
and the small business size standard is $5.0 mil. The solicitation
will be written in accordance with Federal Acquisition Regulation (FAR)
Part 12 for Acquisition of Commercial Items. A site visit is scheduled
for 13 Sep 99 at 1030 a.m. at the base swimming pool, bldg 1620. All
prospective bidders will meet at the Contracting Office, bldg 17000,
prior to proceeding to the site location. Please provide your company
name, list of attendee(s), social security number(s), and phone
number(s) for personnel attending the site visit to the Contracting
Office by 12:00 p.m. local time on 09 Sep 1999. The provision at FAR
52.212-1, Instructions to Offerors _ Commercial Items, applies to this
acquisition. Paragraph (b) is amended as follows: Offerors are
required to submit a signed SF 1449 along with their proposals.
Paragraph (b)(10) is amended as follows: Past performance information
is included as an evaluation factor. Offers shall submit past
performance information to include 3 recent and relevant contracts for
the same items. Include short description, contract no. and point of
contact. The provision at 52.212-2, Evaluation _ Commercial Items,
applies to this acquisition. For the purposes of completing paragraph
(a) of provision 52.212-2, the specific evaluation factors to be used
are (1) product conformity to the specifications (2) price and (3) past
performance. Product conformity and past performance, when combined,
are approximately equal when compare to price. The provision at FAR
52.232-15, Progress Payments Not Included, applies to this acquisition.
Offerors are required to include a completed copy of the provision at
52.212-3, Offeror Representations and Certifications _ Commercial Items
as well as Alternate I and III of this provision. Offerors are required
to include a completed copy of the following Representation and
Certification DFARS provision at 252.212-7000, Offeror representations
and certifications _ Commercial items, and 252.225-7000, Buy American
Act _ Balance of Payments Program Certificate. The clause at FAR
52.212-4, Contract Terms and Conditions _ Commercial Items, applies to
this acquisition. Addendum to FAR 52.212-4, paragraph (o), titled
_Warranty_ is amended to read as follows: The Contractor warrants and
implies that the items/services delivered hereunder are free from
peeling or wearing away in accordance with the Statement of Work in
this contract for a period of five (5) years from the date the service
is completed. Addendum to FAR 52.212-4, to add paragraph (t), FAR
52.252-2, Clauses Incorporated by Reference. The FAR and DFAR clauses
contained in this RFP can be viewed in full text at the following

paragraph (u), Wage Determination. Service Contract Act Wage
Determination, No. 94-2147, Revision No. (10), dated 06/01/1999, is
applicable to this solicitation. The clause at FAR 52.212-5, Contract

at DFARS 252.204-7004, Required Central Contractor Registration,
applies to this acquisition. Please ensure your fax proposals are
marked with RFP number, closing date and time. Offers from sources on
Guam shall include Guam Gross Receipts Tax (GRT) in their unit prices.
Commercial specifications must be included with the proposal.
Proposals are due by 4:00 p.m. local time, on 16 Sep 99, which is 11:00
p.m., Pacific Daylight Time (PDT), on 15 Sep 99. (Note to offerors _
Guam is 17 hours ahead of PDT.) All responsible small business
