The Navajo Area Indian Health Service is soliciting proposals from interested providers to provide on-site mobile mammography diagnostic and screening services for Navajo Area Indian Health Service medical facilities as follows: Tuba City Indian Medical Center, Tuba City, Arizona; Kayenta Health Center, Kayenta, Arizona; Fort Defiance Indian Hospital, Fort Defiance, Arizona; and Chinle Comprehensive Health Care Facility, Chinle, Arizona. The Contractor shall provide all necessary personnel, travel, per diem, medical equipment & supplies, laboratory services, medical liability insurance, etc. The Contractor shall also provide interpretation and transcription associated with the mammography studies. The actual site visit dates shall be coordinated with each Service Unit Site Coordinator. The Contractor's staff must be certified/licensed by the appropriate State or Medical Boards. The resultant contract will be a requirements type contract to be effective for an initial ten (10) month period with two (2) one (1) year option periods.
