This is a synopsis to provide rental and maintenance on the Baker cell #103 prescription units located at the main USAF Academy Hospital, Community Center, and the Peterson AFB Clinic Pharmacy, to replace, add and repair any part of the system when needed. This solicitation is for commercial items prepared in accordance with the format in Federal Acquisition Regulations document (FAR) Part 12 and Part 13, as supplemented with additional information included in this notice. Solicitation number F05611-00-Q0047 is being issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 97-13. All applicable provision and clauses are included in the actual solicitation that can be found at http//www.eps.gov under EPS FOR VENDORS, USAF OFFICES, DIRECT REPORTING UNITS-LOCATIONS, 10th ABW/LGC-POSTED DATE. The government intends to issue a purchase order for the rental and maintenance on the Baker cell #103 prescription system in accordance with the Statement of Work. This solicitation has been approved as a Sole -Source Requirement for Automated Prescription Systems Inc., 4333 Shreveport Highway, Pineville, LA 71360. However, all interested parties may provide a quote with documentation to support the ability to provide maintenance on the above referenced items. A written statement of work will be provided to all interested parties upon request. Offers are due by 20 Sep 1999; quotes must be valid for 30 calendar days after close of solicitation.
