MODIFICATION OF SUBMISSION NO. 377101 POSTED ON SEPTEMBER 7, 1999. The purpose of this modification is to revise Contractor Insurance requirements for this job. The following insurance information MUST be submitted with your quotation. Insurance requirements are hereby revised to read as follows: COVERAGE MINIMUM LIMITS OF LIABILITY 1. Comprehensive Automobile Liability Insurance to include non-owned, hired, or rented vehicles as well as owned vehicles: (i) Bodily Injury $1,000,000 for each person $1,000,000 aggregate for each occurrence (ii) Property Damage $1,000,000 for each accident $1,000,000 aggregate 2. Workman's Compensation for the Statutory Limits State of Virginia and Occupational $500,000 Diseases Employer's Liability Comprehensive General Liability Insurance (including contractual Liability): (i) Bodily Injury $1,000,000 for each person $1,000,000 aggregate (ii) Broad form Property Damage $1,000,000 for each occcurrence $1,000,000 aggregate 3. Excess Liability Not less than $2,000,000 for each occurrence and in the aggregate Contractor shall also carry such additional insurance as may be required by the laws of the State or District. Contractor shall keep the insurance required by this paragraph in full force and effect until acceptance of his work by the Tenant. If Contractor shall subcontract any of his work, Contractor shall be responsible for seeing that such subcontractor shall furnish Owner evidence thereof before the subcontractor commences work onthe project. The insurance shall contain statements of contractual liability or Contractor's indemnity or Contractor's obligations as required and shall in no manner lessen, diminihs, or affect Contractor's obligations set forth in any provisions of the Rules of the Site. Evidence of the above coverage shall be provided by means of two (2) separte Certificates of Insurance. One with "V-Dulles West LLC" as the certificate holder and the other with "Dulles West II Associates, LLC" as the certificate holder. The Certificate of Insurancefor "V-Dulles West LLC" shall include the following two statements: 1. V-Dulles West LLC, as Owner, V-DW Manager, LCC, as Asset Manager, Velsor Properties, LLC, as Asset Manger and Transwestern Carey Winston, LLC, as Manger are additional insured on all policies other than Worker's compensation. 2. A waiver of subrogation in favor of (Ownership Entity) is included on all policies. The Certificate of Insurance for "Dulles West II Associates,LLC" shall include the following two statements: 1. Dulles West II Associates, LLC, as Owner, V-DW Manager II, LCC, as Asset Manager, Velsor Properties, LLC, as Asset Manger and Transwestern Carey Winston, LLC, as Manger are additional insured on all policies other than Worker's compensation. 2.
