Y -- CONSTRUCTION OF STRUCTURES AND FACILITIES -- MODULAR LABORATORY DUE 092999 POC James Reuter (513) 648-5166 Fluor Daniel Fernald (FDF) and the Department of Energy (DOE) have prepared and approved a plan to construct a 4872 square foot modular laboratory at the Fernald Environmental Management Project (FEMP) located in Fernald, Ohio. In addition, the modular laboratory facility will utilize three existing DOE owned road transportable analytical laboratories (RTAL's) to provide analytical service to the site during final remediation activities. Work will include but is not limited to: site clearing and grubbing; excavation and installation of underground piping; foundation installation; design, system, fume hoods, case work, utility piping, and countertops; fire alarm installation, HEPA filters; Compressed air system; 13.2KV overhead electrical extension; communication systems including telephone, evacuation, and computer; utility piping installation; ramps, stairways, and walkway installation. Potential proposers must meet the pre-qualification criteria listed below; 1. Proposers or lower tier subcontractor must possess (5) years experience in the set-up and installation of modular structures. 2. The proposer must possess (5) years experience in the performance of government contracts ranging in price from 1-5 million dollars. 3. The proposer or lower tier subcontractor must be familiar with excavation and installation of foundations, sanitary sewer, lift stations, and domestic water. 4. The proposer or lower tier subcontractor must possess (5) years experience in the installation of 13.2KV overhead electric, transformers, pole lines, disconnects, fire alarm, and communications systems. 5. The proposer or lower tier subcontractor must possess (5) years experience in the design, fabrication, and installation of modular laboratory buildings including HVAC design, fume hood ventilation, case work and finishings, specification, HEPA Filter and installation of mechanical and utility systems. 6. Safety Performance Information A. Proposers and their lower tier subcontractors shall have a record of good safety performance to be determined responsible and, therefore, eligible for any award on projects managed by Fluor Daniel at the Fernald Environmental Management Project in Fernald, Ohio. B. To be considered responsible, a proposer must be able to demonstrate to the satisfaction of Fluor Daniel that it has a record of safe performance as demonstrated by a current workers compensation insurance experience modification rate (EMR) of less than or equal to 1.00; or the trend for the past three years is downward and none of the past three years had an EMR greater than 1.15; or for proposers that are too small, too new or self-insured, has an OSHA recordable incident rate of less than or equal to 10 injuries and illnesses per 200,000 man hours for the past calendar year. Each proposer shall submit the information required with their qualifications. C. Firms who cannot meet the safety rates or special criteria described in paragraph B above, but nevertheless desire to submit their qualifications may do so on the following conditions: 1. Include a narrative statement as to the reason(s) for not being able to meet the criteria and request Fluor Daniel consideration of any special circumstances that relate to its rating. 2. Fluor Daniel will evaluate the firm's request for special consideration and acceptability of the bid. 3. The firm (proposer) agrees that the Fluor Daniel evaluation and decision as to acceptability for this solicitation is final and is not subject to discussions of any type between Fluor Daniel and the proposer and is not subject to any appeal or dispute.
