a combined synopsis/solicitation for commercial items prepared in
accordance with the Subpart 12.6 as supplemented with additional
information included in this notice. This announcement constitutes the
only solicitation; proposals are being requested and a written
solicitation will not be issued. This contract has been SET-ASIDE for
SMALL BUSINESS Standard Industrial Classification 8099. Description --
The contractor shall provide a qualified dietitian to be responsible
for providing dietetic health and wellness services to all Government
beneficiaries. The Dietitian shall be responsible for planning,
coordinating, implementing, and evaluating Health and Wellness Center
and base-wide nutrition and other health and wellness programs. These
include: developing the nutrition budget, creating continuity files,
implementing instructional material, aids, and techniques, coordinating
activities, classes, and programs, and teaching clients one-on-one or
in group settings. In addition, he/she will conduct nutrition lectures
and workshops on subjects including smoking cessation programs,
fitness programs, drug and alcohol abuse prevention, and parenting
classes. Related topics include healthy lifestyle behaviors, minimizing
health risks, and preventing further disease complications. Activities
to be included are promoting nutrition during health fairs and special
theme events, providing dietary expertise during cholesterol and
diabetes screenings, and overseeing base wide weight management
program. The dietitian will be responsible for interviewing clients
using the Health Enrollment Assessment Review and completing nutrition,
health risk education, and treatment plan. Further, he/she will
instruct clients and family members, using change model stages, on
appropriate diet and/or methods to reduce identified health risks.
He/she will perform a biennial need assessment to determine base-wide
nutritional deficiencies and will provide education based on health
risk, high volume and cost effectiveness to clients. The dietitian will
be responsible for actively marketing nutrition programs through the
newspaper, e-mail, displays, briefings, seminars, and workshops. Last,
the dietitian will work directly with food facility mangers
implementing the Check It Out program and advise both the Child
Development Center and Youth Center nutrition programs. The dietitian
shall be available Monday through Friday, 7am to 4pm September 27, 1999
through September 27, 2000, excluding federal holidays. The government
will conduct a criminal background check. The dietitian must provide
a copy of a valid, current, restricted, state dietetic license and
certificate of registration by the American Dietetic Association.
He/she must demonstrate dietitian education and training and be
certified in current basic life support. Strong writing and verbal
skills must be demonstrated. Certification of current physical
examination is required and must be current as of 12 months prior to
time of application. A Bachelor of Science in Dietetics or nutrition
shall be possessed from an accredited 4-year college or university. In
addition, the dietitian shall have completed an accredited dietetic
internship. He/she shall have provided dietetic services a minimum of
I year in direct patient counseling within the last 3 years. In
addition a sworn Affidavit of truthfulness is required indicating
experience, training, and technical expertise in the type of care to be
rendered. All responsible sources may submit a quotation, which shall
