The Portland Area Diabetes Program (PADP) is updating diabetes education materials. The contractor shall modify design and layout text of booklets and magazines which will contain current information for diabetes prevention and management. Real life stories, from interviews, giving examples of how people can make health choices, including nutrition, and physical activity specific for better diabetes prevention and management. Factual information for diabetes and American Indians. PADP will be the lead for booklet modification and field. The contractor shall make camera ready 8 diabetes education booklets. PADP shall supply text and line art for modification. The contractor will also design and format 2 magazines of approximately 44-60 pages each. Cost includes format, design, story development, layout, editing. Contractor shall supply all needed photos, artwork and text. PADP shall supply factual information on diabetes and American Indians. Contractor shall be responsible for securing all necessary releases for all photo work and interviews.
