synopsis/solicitation for commercial items/services prepared in
accordance with format in sub parts 12.6 and 13.6, as supplemented with
additional information included in this notice. This announcement
constitutes the only solicitation to be issued, proposals are being
requested from this announcement, a written solicitation will not be
issued. The Request for Proposal is 575-502-00. This solicitation
incorporates provisions and clauses in effect through FAC 97-12.
Standard Industrial Code (SIC) of 8999 is applied. This solicitation is
issued subject to the availability of government funds at time of
award. This solicitation is unrestricted for the following service:
Contractor shall provide maintenance for the following General Electric
Medical X-Ray system -- AMX4, Steno/D6-D9, SFX30/MVP, RAD MVP60,
SFX30/ADVTX. Maintenance is to include one preventive maintenance per
year, all parts & labor during business hours (8-5 Monday thru Friday),
on phone service, on line service through the modem, and manufacturer
service software to be included. The following provisions and clauses
are applicable and are hereby incorporated into the solicitation by
reference and will be provided by the contracting officer in full text
upon request. FAR 52.212-1, Instruction to Offerors Commercial Item
Applies. Offerors are required to complete and include a copy of the
following provisions with their proposals: FAR 52.212-3, Offeror
REpresentations and Certifications Commercial Items, FAR 52.212-5
Affirmative Action for Special Disabled and Vietnam Era Veterans; FAR
52.222-36, Affirmative Action for Handicapped Workers; FAR 52.222-37,
Employment Reports on Special Disabled Veterans and Veterans of the
Vietnam ERA. Award will be made to that offeror (1) whose proposal is
technically acceptable and (2) whose technical/cost relationship is
most advantageous to the Government. While cost is secondary to
technical, it will be a factor in the award decision. Also, award may
not necessarily be made for technical capabilities that would appear to
exceed those needed for the successful performance of the work.
