Lakes Acquisition Center intends to negotiate a Sole Source Contract
with Mallincrodt Inc. of Minnesota for purchase of one (1) NPB SANDMAN
NT Bed Data Collection and Analysis System for the VA Hospital, Fifth
& Roosevelt Rd. Hines, IL 60141. This is a combined
synopsis/solicitation for commercial items prepared in accordance with
the format in Far Subpart 12.6 as supplemented with additional
information included in this notice. This announcement constitutes the
only solicitation. Solicitation number RFQ 69D-397-99 has been
assigned to this procurement. Solicitation documents and incorporated
provisions and clauses are those in effect through Federal Acquisition
Circular 97-13. SIC (3845). Cost shall include all labor, material,
tools and equipment necessary to furnish and install (1) One NPB
SANDMAN NT Bed Data Collection and Analysis System and required
accessories as defined herein. Equipment shall be shipped FOB
Destination to the attention of the Chief, Medical Service, VA Hospital
Fifth & Roosevelt, Hines IL 60141 and shall be delivered within 30 days
from receipt of Purchase Order or at a date mutually agreed upon with
the vendor. It is understood that upon receipt of purchase order, a
customer information summary (CIS) will be sent to the COTR for
completion. The CIS Package will assist the VA and vendor on the
installation of the equipment. Upon return of the CIS Package, a final
installation date will be determined and mutually agreed upon with the
vendor Cost must include all shipping, handling, on-site training and
all costs necessary or incidental for a complete delivery and
installation. Vendor shall ensure that the equipment functions in
conformance with the latest published edition of NFPA-99, UL, OSHA,
JCAHO, VA and applicable standards. The equipment MUST be compatible
and be fully integrated with the existing Sandman NT, software and
archiving systems at the VA Hospital. Procurement shall include but not
be limited to the following: One (1) Item No. D.0148 NPB Sandman NT,
Bed Data Collection and Analysis System; One (1) D.0135 Sandman NT, Bed
Collection and Analysis; Two (2) D.0141 Sandman NT Bed Collection
Add-On; One (1) Viewer Software Only (MD Review); Four (4) D.0132
MMC-16 Channel Amplifier Kit Series III Including amplifier 25, 40, 60,
80 or 100' cable, 1 bed; Five (5) D.3208 21" Hi-Resolution Monitors;
Five (5) D.3109 Desktop Computers; One (1) D.0055 Color Printer Kit;
One (1) D.0121 Data Archive Combo Jaz and CD Recorder; One (1) D.0124
Five Computer Network (NT) Systems; Four (4) D.0066 Adult Electrode
Starter Kits; Four (4)D.5018 NPB N-295 Pulse Oximeter; Four (4) D.5109
ProTech Body Position Sensor; Four (4) D.4043 APC BK-UPS 650V; Four
(4) D.4005 Anthrocart including three shelves. Software shall include
all NT Sandman Software, Data Acquisition Board, Remote Support Package
(PC Anywhere, Zip Drive and External Modem) and Microsoft Office 97
Small Business Edition to allow for report editing. Vendor shall
provide on-site training for three (3) days minimum forthree (3) people
on the Sandman only; Sandman Standard Reports including up to two (2)
days customization and two (2) copies of operation/user's manuals
furnished to the Hospital upon completion of installation; Cost shall
include all required computers, software and/or hardware licenses and
peripheral devices necessary to operate the equipment. It is the VA's
intention to purchase the equipment as a turnkey project. Each of the
five (5) computers will be required to operate the system and shall
have the following minimum characteristics: Premium Desktop Dell
Optiplex GX1/MT+ Computer, with P450MHz side Bus 100MHz, Pentium II
Processor, 512KB Cache, 6.4GB Hard Drive, 64MB RAM, 1.44 Mb Floppy, CD
ROM, keyboards, Intellimouse, Internal 10/100MB 3COM Network Cards,
ATI Rage Pro Turbo (8MB) Video Card, Windows 95; Windows NT v4.0, and
Workstation. All computers shall come complete with a three (3) year,
next business day on-site service guarantees. This will be a firm fixed
price contract AND must include all costs associated with the delivery,
hardware/software, training and installation of the equipment. The
Sandman NT requires the use of proprietary software and to the best of
the government's knowledge, Mallinckrodt, Inc. is the only available
source, which can provide this product as defined herein. License
agreement and unlimited use of the software is included. Vendor shall
provide a copy of the written warranty with quote and all
warranties/guarantees shall be in effect for the specified time period
from the date of final acceptance by the Government or equivalent to
what is provided in the commercial market. Unless otherwise specified
the Sandman NT and associated products will conform to NPB's
performance specifications and will be free from defects in material or
workmanship for 12 months following acceptance. If, during the warranty
period, the product does not perform as warranted, or if it is not free
from defects and materials or workmanship and has not been misused
(i.e. dropped) or subjected to abnormal conditions, the vendor at its
own expense, will repair or replace such non-conforming or defective
equipment. Parts considered expendable or disposable are not covered by
the warranty. (I.e. diskettes, electrodes, printer ribbons, etc.). COTR
for the project shall be Mr. James Fink. All equipment, replacement
parts, components, hardware, software and firmware containing
information technology delivered under this contract shall be Year 2000
Compliant. Year 2000 Compliance means that information technology
accurately processes date/time data (including, but not limited to,
calculating, comparing, and sequencing) from, into, and between the
twentieth and twenty-first centuries, and the years 1999 and 2000 and
leap year calculations. The Vendor is required, as part of the offer,
to provide documentary evidence that equipment, replacement parts,
components, hardware, software and firmware offered is Year 2000
Compliant. The following incorporated provisions and clauses are those
in effect through FAC 97-13 and are applicable to this acquisition.

acceptance by the Government. The proposed contract action is for the
purchase and installation of this equipment, which the Government
intends to solicit and negotiate with only one source under the
authority of FAR 6.302-1. All FAR provisions and clauses may be
reviewed and/or obtained from the General Service Administration's
Federal Acquisition Regulation website at Internet address
http://www.arnet.gov/far. This notice of intent is not a request for
competitive proposals. A determination by the Government not to compete
this proposed contract, based upon responses to this notice, is solely
with the discretion of the Government. Information received will
normally be considered solely for the purpose of determining whether to
conduct a competitive procurement. All responses must be submitted no
later than 2:00 PM on 09/24/99 to the Contracting Officer indicated
