(703) 695-5185 CBD COMBINED/SYNOPSIS SOLICTATION (i) This is a combined
synopsis/solicitation for commercial items prepared in accordance with
the format in Subpart 12.6, as supplemented with additional
information included in this notice. This announcement constitutes the
only solicitation; proposals are being requested and a written
solicitation will not be issued. (ii) This Solicitation MDA94699T0604
is a Request for Quote (RFQ) for medals and badges as described herein.
(iii) This Solicitation document and incorporated provision and clauses
are those in effect through Federal Acquisition Circular Volume 64, No.
116, dated 17 June 1999. (iv) This procurement is 100% set-aside for
small business firms. The applicable Standard Industrial Code is 8400
and the corresponding size standard is $5.0 million. The Small Business
Competitiveness Demonstration Program does not cover this procurement.
(v) & (vi) DESCRIPTION OF THE REQUIREMENT: The contractor shall
provide a quote to provide: CLIN 0001: Defense Distinguished Service
Medal Set, set includes medal, ribbon, lapel pin and box, quantity 200
sets; CLIN 0002: Badge, Secretary of Defense: Anodized -- a silver
sunburst of 33 rays and gold annulet with 13 stars on which is imposed
an eagle with a shield on its breast grasping 3 crossed arrows. CLIN
0002AA: Badge, small size (1-l/2"), quantity 1,000 each; and CLIN
0002AB: Badge, large size (2"), quantity 1,000. (vii) Date(s) and
place(s) of delivery and acceptance and FOB point: Delivery is required
as soon as possible after receipt of the award document; a partial
shipment will be acceptable (50%). The ship to address is the Pentagon
Building, Washington, D.C. (viii) The following provision applies to
this acquisition: Federal Acquisition Regulation (FAR) 52.212-1
entitled Instructions to Offerors -- Commercial. The following
provision is being added as an Addendum: FAR 52.214-31 Facsimile
Bids/Quotes. (ix) The following provision apply to this acquisition:
FAR 52.212-2 entitled Evaluation -- Commercial Items. The Government
will evaluate the offers received based on delivery time and price
(factors equal in importance). The Government intends to review quotes
and award a contract without discussions. The Government reserves the
right to conduct discussions if later determined by the contracting
officer to be necessary. The Government may reject any or all quotes if
such action is in the public interest; and waive informalities and
minor irregularities in offers received. Award will be based upon best
value to the Government. (x) Offers are to include a completed copy of
provision FAR 52.212-3 entitled Offeror Representations and
Certifications -- Commercial Items. (xi) The following provisions apply
to this acquisition: FAR 52.212-4 entitled Contract Terms and
Conditions -- Commercial Items and FAR 52.243-1 entitled Changes --
Fixed Price. (xii) The following clause is applicable to this
acquisition: FAR 52.212-5 entitled Contract Terms and Conditions
Required To Implement Statutes Or Executive Orders -- Commercial Items.
The following FAR Clauses within the aforementioned clause are being
selected as applicable: 52.222-21, 52.222-26, 52.222-35, 52.222-36,
52.222-37, 52.225-3, 52.225-9, 52.225-18, 52.232-33. (xiii) DFAR
252.204-7004 entitled Required Central Contractor (CCR) Registration is
applicable to this requirement. Vendors may obtain CCR information by
calling 1-888-227-2423 or via the Internet at http://ccr.edi.disa.mil.
ONLY FIRMS REGISTERED IN CCR ARE ELIGIBLE FOR CONTRACT AWARD. (xiv)
There is not a Defense Priorities and Allocations System (DPAS) number
assigned to this acquisition. (xv) See Numbered Note 1. (xvi)
