Murphy 612-725-2190 Under Statutory Authority 41 USC 253(c)(1) the
Government anticipates entering into a brand name only procurement of
Howmedica Osteonics, Rutherford, NJ Surgical Bone Cement, number
6191-1-010. This contract is for Brand Name Only supplies and is hereby
issued for commercial items to be used at the VA medical center and is
prepared in accordance with the format in FAR Subpart 12.6 and FAR
Subpart 13.5, as supplemented with additional information included in
this notice and incorporates provisions and clauses in effect through
FAC 97-13. The authority is cited and justification made based on the
determination that the Government's minimum needs can best be satisfied
by Brand Name Only supplies. The SIC code for this procurement is 3841
and the small business size standard is 500 employees or less. This
CBD announcement constitutes the only solicitation issued. A separate
written solicitation document will not be issued. Contractor's
proposals are hereby being requested in response to requirements for
brand name only products listed below and shall be submitted on
contractor's letterhead or bid form and must address all requirements
listed and must contain the name, title, and signature of person
authorized to submit proposals/bids on behalf of the contractor. VA
Medical Center, Minneapolis, MN has a requirement for brand name or
equal Howmedical Osteonics, Rugherford, NJ. Item One -- 660 each --
Surgical Bone Cement, #6191-1-010, NSN 6515-01-239-1535. This is a
requirements contract for Fiscal Year 2000, with two 12-month options
to renew for Fiscal Year 2001 and 2002. Price proposal will include
separate price schedules for each Fiscal Year. The VA is requiring FOB
destination pricing. All supplies provided will be Y2K compliant. All
responsible sources who can meet the VAMC's requirements and provide
items as listed above may respond to this solicitation by submitting a
price proposals on their company letterhead or bid form. Evaluation
factors will include, but may not be limited to, delivery,
pastperformance and price/price related factors. Products will be
delivered either to the VA Medical Center, Warehouse (90D), One
Veterans Drive, Minneapolis, MN 55417 or to the Medical/Surgical
Product Prime Vendor. The solicitation clauses incorporated herein are
those in effect through FAC 97-13 and are applicable to this
