SYNOPSIS/SOLICITATION for commercial items in accordance with the
format in Federal Acquisition Regulation (FAR) Subpart 12.6, as
supplemented with additional information included in this notice. This
announcement constitutes the only solicitation for this requirement;
proposals are being requested and a written solicitation will not be
issued. Solicitation Number FOH-42(9) is issued as a Request for
Quotation, under simplified acquisition procedures, test program,
unrestricted. All responsible sources may submit a proposal. All timely
proposals will be considered by the Government. The SIC code is 3845
and the small business size standard is 500 employees. The Government
intends to award an Indefinite-Delivery-Indefinite-Quantity type
contract for a base year with a one year option period to the
responsible offeror whose total offer for all line items is evaluated
and determined to be the best value for the Government, price and other
factors considered. Award shall be on an "all ornone basis". The
solicitation document and incorporated provisions and clauses are those
that are in effect through Federal Acquisition Circular 97-13. The
following is a description of the requirements for this acquisition:
The purpose of this requirement is to acquire Automatic External
Defibrillators (AED's) and associated supplies for use in FOH
occupational health clinical service sites and in other Federal
buildings located throughout the United States, Pacific Islands, Virgin
Islands, and Puerto Rico. Following are the AED equipment requirements:
The Basic AED Unit shall be: Y2K compliant -- with manufacturer
warranties as to compliance; Bi-phasic waveform; Voice prompting;
Visual display with back lighting; Visual prompting; Automatic
self-testing; Removable data capture media -- i.e., data card; Data
media capable of capturing voice, ECG, electronic events and rhythm
analysis; ECG display model (if available). Each AED shall be shipped
and set up with the following accessories: Disposable lithium
batteries/battery pack; Disposable defibrillation pads/electrodes;
Electrode adapter; Data card/data recording media; Carrying case;
Instruction booklets; Quick reference booklets; Training video/user
guide; Data media patient labels; and Manufacturer's technical support
available. Data media interpretation computer software with users
guide shall be available separately. An AED Training Unit shall also be
provided and equipped with the following training accessories:
Pre-programmed training scenarios; Training video; Instruction
booklets; Disposable training pads/electrodes; and Carrying case for
accessories. Prices shall be F.O.B. Destination. Individual Delivery
Orders shall specify place of delivery, which shall be FOH sites and
other Federal buildings located throughout the United States, Pacific
Islands, Virgin Islands, and Puerto Rico. The guaranteed minimum number
of units that the Government plans to order for each contract period is
25 each, and the maximum number is 500 each. At the time of contract
award, the Government will designate an authorized individual who will
place delivery orders under this contract. Contractors shall complete
the unit price for all line items for all contract periods. Quantity
discounts should be provided utilizing a separate Pricing Schedule
annotating the quantity limits applicable to the proposed discount. If
quantity discounts are proposed, the Government prefers that the
quantity discount be based upon the cummulative number of units placed

FAR 52.212-2, Evaluation-Commercial Items-Evaluation factors include
(a) technical capability of the contractor to meet the requirements of
this solicitation and (b) price. Technical and price criterion are
considered to be equal. Proposals submitted in response to this
combined synopsis/solicitation, shall include current product
literature and a copy of the Offeror's most recent commercial price
list. Proposals shall be submitted in an original and two copies. FAR
