Bonner, 440.838.6091 This is a combined synopsis/solicitation for
commercial supplies/services prepared in accordance with the format in
FAR Subpart 12.6 as supplemented with additional information included
in this notice. This announcement constitutes the only solicitation;
Invitation For Bids (IFB) are being requested and a written
solicitation will not be issued. Solicitation document and incorporated
provisions and clauses are those in effect through Federal Acquisition
Circular Number 70. This procurement is set-aside for small businesses
and the SIC Code is 5713 and the small business size standard is $ 5.0
million. The Contractor shall perform the services identified herein
at the Louis Stokes Cleveland, Veterans Affairs Medical Center, 10000
Brecksville Road, Brecksville, Ohio 44141, Building 5. Services shall
be performed during the hours of 8:00 a.m. to 4:30 p.m., Monday through
Friday. Services performed other than the hours specified shall be
pre-arranged with the Contracting Officer. Item (1)
Supplies/Servicesare required to furnish and install Lee's MBH
collection carpet, pattern: DQ586-207 by direct glue down method with
7" matching carpet base. The carpet specifications are as follows:
Construction: Tufted; Surface Texture: Performance Textured Loop;
Gauge: 1/8 inch; Stitch Count: 8.8 per inch; Pile Height: .187 inch
average; Face Yarn: Antron Legacy Nylon with Dura Tech soil protection
by DuPont; Dye System: Yarn dyed DuraColor by Lee's stain resistant
system; Face Weight: 26 ounces per square yard; Backing construction:
Unibond by Lee's; Primary Back: Woven Polypropylene; Secondary Back:
Woven Polypropylene; Total Weight: 77.10 ounces per square yard; Width:
12 feet; Thermobond 6 by Lee's. Approximate floor space is 610 square
yards. Carpeting shall be installed in all areas of 51A including room
107 which is in the center area, outside of ward 51A. Excluded are the
corridors, rooms 110, 113 and 125, and the rooms containing ceramic
tile such as the restrooms, shower rooms, utility rooms, etc.
Installation shall be completed by close of business October 22, 1999.
Item (2) Supplies/Services are required to furnish and install Lee's

ward 51A. Installation shall be completed by close of business October
22, 1999. Item (3) Supplies/Services are required to furnish and
install Interface Caribbean II and Interface Details 6" Performance
Broadloom with matching 4" carpet base. The carpet specifications are
as follows: Yarn System: 67% DuPont Antron Lumena, 33% duPont Antron
Legacy; Color System: 67% Solution dyed, 33% yarn dyed; Lifetime
Anti-Microbial: Intercept; Soil and Stain Protection: Protekt with
DuPont Dura Tech; Pile Thickness: .138M 3.5mm; Tuft Gauge: 1/12; Pile
Density: 6.261; Weight Density: 150.261, C-1 Interface Caribbean needed
for approximately 165 square yards of floor space; C-2 Interface
Details needed for approximately 58 square yards of floor space. The
Contractor shall remove the existing carpeting and furniture in all of
the rooms within the suite of the Mental Health Line -- Office of the
Chief, and, make any floor preparations that are required. The
Contractor shall move the furniture back into all of the rooms once the
carpet has been installed. Installation shall be completed by close of
business November 24, 1999. The Offeror shall provide separate pricing
for items 1,2 and 3 and each item shall have separate pricing, pricing
for the carpet shall be per square yard, and the carpet installation
shall be 1 job for each item, for item 3 floor preparation, removal of
carpeting and moving of furniture shall have separate pricing. A site
visit will be held on September 22, 1999 at 10:00 a.m., Offeror's will
meet the Contracting Officer in the lobby of building 5. The following
provisions apply to this acquisition: 852.219-70 Veteran-Owned Small
Business; 852.270-4 Commercial Advertising; 52.212-1 Instructions to
Offerors-Commercial Items; 52.212-3 Offer Representations and
Certifications-Commercial Items 52.212-4 Contract Terms and
Conditions-Commercial Items; 52.212-5 Contract Terms and conditions to
Implement Statutes of Executive Orders-Commercial Items, including
clauses incorporated by reference under Section (b) 11 through 16 and
20. Offerors responding to this announcement shall provide the
information required in provisions 52.212-1, a completedcopy of
52.212-3 and the certification required in 852.219-70. Invitation for
Bids shall be received by September 27, 1999 at 10:30 p.m. local time.
