This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation 626-99-40-jo is issued as a Request for Quote (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-12: FAR 52.212-1 Instructions to Offerors; FAR 52.212-4 Contract Terms and Conditions Commercial Items; FAR 52.212-5, Contract Terms and conditions Required to Implement Statutes or Executive Orders- This acquisition is unrestricted. SIC is 3841 and small business size standard is 500 employees. This requirement is for an Electrophysiology (EP) Laboratory System for our Electrophysiology Laboratory, and the system must have real-time RF ablation display capability, image management capabilities and a stimulator. The minimum requirements for this system are as follows: (1) 32 Channel EP Laboratory System that will consist of at a minimum (a) System Controller, Amplified Unit, Two 21" flat screen 1600x1280 color monitors, HP or equal LaserJet with 600dpi capability, Program software, Workstation Cart, minimum of 33.3 Baud modem, spare fiber optic cable and catheter input modules, Four 650MB optical disk cartridges, Isolation safety transformers and UPS and other appropriate accessories and manuals; (2) 75ft db 15-5bnc cable to connect monitor to workstation; (3) RF Ablation Device Interface Module hardware and software for recording/displaying real-time information, (state what RF ablation units this will retrieve information from); (4) 48 Channel Upgrade from 32 Channels; (5) Four channel High Level A/D Board (allows the EP system to input up to at least 4 high level analog signals from external devices such as RF ablation systems and device programmers; (6) Pressure Transducer Adapter for EP Lab Amplifier;(7) Adequate training and in-service as determined by VAMC, Nashville; (8) Adequate training for one nurse/technician that will include tuition, lodging, and airfare; (9) Minimum of one year warranty on all hardware, Ninety days on cables and Software maintenance updates for life of equipment.
