OLYMPUS AMERICA BORESCOPE _ This is a combined synopsis/solicitation
for commercial items prepared in accordance with the format in Subpart
12.6, as supplemented with additional information included in this
notice. This is a sole source acquisition to Olympus America Inc., out
of Lake Success, NY. The work is for the procurement of an Olympus
America borescope kit. PLEASE READ THIS NOTICE CAREFULLY AS IT
CONSTITUES THE ONLY WRITTEN SOLICITATION THAT WILL BE ISSUED.
Solicitation number F65501-99-Q0329 is issued as a Request for
Quotation (RFQ). The incorporated document provisions and clauses are
those in effect through Federal Acquisition Circular 97-12. The
Standard Industrial Classification (SIC) Code is 3827. The small
business standard is 500 employees. LINE ITEM 0001, (1) EACH, ILV-C1
SYSTEM CASE, P/N: 1LV-C1/IV6A/DSM/C, MODEL 7030A, Grey, molded, outer
shell case for ILV-C1 light source and remaining RVI equipment.
Olympus_ most advanced and diverse (multiple configurations) system
case. Durable system case can be _checked_ at the airport (internal
equipment mounted via rubber isolators). LCD display mounted on a
telescoping pole for easy deployment and storage and adjustable viewing
levels. Padded compartment for quick and easy storage of scope and
insertion tube. Accessible video-out cables to connect to a VCR or
external monitor (BNC or S-Video). Removable top lid for easy
maneuverability in tight workplaces and removable bottom lid for easy
cable access. Weight: 9 lbs. Size: 21_(Width) x 19_ (High) x 8.5_
(Depth). IV-6A CCU: Camera control unit (CCU) for all Series 6
Videoscopes and the OTC-6 Fiberscope/Borescope camera. Includes menu
driven features such as: high gain: 1 to 9.9 integration 1/60 sec to 10
sec; automatic CCD shuttering, 1/10,000th to 1/60th sec.; second; white
balance with memory feature; live audio signal; live pan and zoom;
image enhance; date and time display. Size 6.8_ (Width) x 2.3_ (High)
x 9,7_ (Depth). Weight: 3.3lbs. Power input 12 VDC (via optional AC
Adapter at 100-240 VAC 50/60 Hz or ILV-C1). DSM-2: The DSM-2 is a
small, lightweight portable custom embedded PC system that provides
image freeze, storage, import/export, measurement and audio annotation
capabilities that easily integrates with the IV Series Videoscopes.
Freeze frame, stores 20 images with audio clip internally,
import/export in JPG and IW-2 formats (up to 200 images externally
stored using SmartMedia _Flashpath_), measuring using IV6C^ & IV8C6
Stereo Measurement, 2D linear scalar or DMB. OVD-2B LCD Display with
Sunshade: 6_ LCD scope mount display to be used with 8mm or 6mm C6
Series IV scopes. Accepts composite (NTSC) Video only. IV-6/6A Audio
Cable: Internal Communication Cable, IV6/6A Audio Cable enables
connection to the audio terminals of a VTR, etc. Cable set (five cable
set) to be used with the ILV-C1 System Case, ILV-C1 light source,
IV6A/IV6 CCU and DSM-1. Cable Assembly for OVD-2B: Cable Assembly for
the ILV-C1 lightsource that supplies power and video to the OVD-2B.
Delivery terms shall be FOB destination. Delivery shall be made NLT 90
calendar days from receipt of order. Delivery shall be made to the
following address: 3 CRS/LGMP, 8549 Sijan Ave., Elmendorf AFB, AK,
99506. The following provisions and clauses are incorporated and shall
remain in full force in any resultant purchase order; FAR Part
52.212-1; Instructions to Offerors, Commercial Items is incorporated by
reference. The provision at 52.212-2; Evaluation-Commercial Items,
applies to the solicitation. Award will be made based on
low-price/technically acceptable offeror. The government reserves the
right to make award on the initial quote received without discussions.
Each offeror shall include a completed copy of the provision at FAR
52.212-3; Offeror Representations and Certifications -- Commercial
Items with its quote. FAR Part 52.212-4; Contract Terms and
Conditions-Commercial Items, is incorporated by reference and applies
to this acquisition. Note the following addenda to FAR 52.212-4:
Incorporate DFARS 252.204-7004; Required Central Contractor
Registration. Incorporate the clause by reference at 52.212-5; Contract
Terms and Conditions Required to Implement Statutes or Executive Orders
-- Commercial Items. However, for paragraph (b) only the following
clauses apply: 52.222-21, Prohibition of Segregated Facilities;
52.222-26; Equal Opportunity, 52.222-35; Affirmative Action for
Disabled Veterans and Veterans of the Vietnam Era, 52.222-36;
Affirmative Action for Handicapped Workers with Disabilities,
52.222-37; Employment Reports on Disabled Veterans and Veterans of the
Vietnam Era, and 52.247-64; Preference for Privately Owned U.S. --
Flag Commercial Vessels. Incorporate DFARS 252.212-7001, Contract Terms
and Conditions Required to Implement Statutes or Executive Orders
Applicable to Defense Acquisitions of Commercial Items. However, for
paragraph (b), only the following clause applies 252.225-7001, Buy
American Act and Balance of Payments Program. The Contractor shall
extend to the Government their standard warranty offered insimilar
types of sales. Acceptance of the warranty does not waive the
Government_s right with regard to the other terms and conditions of the
contract. In the event of a conflict, the terms and conditions of the
contract shall take precedence over the warranty. The warranty period
shall begin upon final acceptance of the items listed in the schedule.
This is a DO rated order with a priority identification symbol of C9E.
See CBD Note 22. A firm-fixed price purchase order will be issued under
the Simplified Acquisition Procedures. Submit signed and dated offer to
3 CONS/LGCS, 10480 22nd St., Elmendorf AFB, AK 99506, at or before 1530
(3:30 p.m.) Alaska Time 27 Sep 99. Quotation shall be signed, dated and
submitted on your company_s letterhead or quotation forms. Quotations
may be submitted via fax. All questions concerning this Amendment must
