on August 25, 1999 has been amended. Amendment A00007 is issued to the
above combined synopsis/solicitation for purchase of Employee
Assistance Program services. This amendment is issued in accordance
with FAR Subpart 12.6, as supplemented with additional information
included in this notice. This announcement constitutes an amendment of
the above solicitation, proposals are being requested, and a written
amendment will not be issued. Pursuant to FAR 15.206, this amendment is
issued to add a term and condition to the combined
synopsis/solicitation posted to the CBDNet on August 25, 1999, but does
not constitute a material change in the Government's requirements. As
a result of this addition, THE SOLICITATION CLOSING DATE IS REVISED.
Under APPLICABLE CLAUSES & PROVISIONS, FAR 52.212-2, "Evaluation
Commercial Items (Jan 1999)," is hereby added as follows: 52.212-2
Evaluation -- Commercial Items (Jan 1999) (a) The Government will award
a contract resulting from this solicitation to the responsible offeror
whose offer conforming to the solicitation will be most advantageous
to the Government, price and other factors considered. The following
factors shall be used to evaluate offers: 1. Past Performance 2.
Technical Capability 3. Price Under Past Performance, the Government
will evaluate offers for award purposes considering the following
sub-factors: quality of service, timeliness of performance, cost
control, and business practices and customer satisfaction. Under
Technical Capability, the Government will evaluate offers for award
purposes considering the following sub-factors: personnel
resources/staffing approach and key personnel. Past Performance and
Technical Capability are equal in importance. Technical Capability and
Past Performance, when combined, are higher than Price. If two or more
proposals are rated as close to equivalent in Technical Capability and
Past Performance combined, the evaluation of price will become a more
significant factor in theaward decision. (b) Options. The Government
will evaluate offers for award purposes by adding the total price for
all options to the total price for the basic requirement. The
Government may determine that an offer is unacceptable if the option
prices are significantly unbalanced. Evaluation of options shall not
obligate the Government to exercise the option(s). (c) A written notice
of award or acceptance of an offer, mailed or otherwise furnished to
the successful offeror within the time for acceptance specified in the
offer, shall result in a binding contract without further action by
either party. Before the offer's specified expiration time, the
Government may accept an offer (or part of an offer), whether or not
there are negotiations after its receipt, unless a written notice of
withdrawal is received before award. THE DATE FOR RECEIPT OF PROPOSALS
IS HEREBY EXTENDED TO OCTOBER 8, 1999, 3:00 P.M., LOCAL (WASHINGTON,
DC) TIME. AMENDMENT ACKNOWLEDGEMENT: Proposals must acknowledge receipt
of this amendment before the hour and date specified in the
synopsis/solicitation. Failure of this acknowledgement to accompany the
proposal may result in rejection of your proposal. If as a result of
this amendment you desire to change a proposal already submitted, such
change may be made by a letter. The letter must refer to the RFP No.
HQ-99-06 and this amendment, and be received before the time and date
specified. The synopsis/solicitation HQ-99-06 is amended as indicated
above. The hour and date for receipt of proposals is extended to
October 8, 1999, 3:00 P.M. local (Washington, DC) time. Except as
provided herein, all other terms and conditions of HQ-99-06 remain
unchanged and in full force and effect. End of amendment A00007 to
