Lujan, Contracting Officer, (505) 248-4565 The Albuquerque Area Indian Health Service intends to issue a Request for Proposal (RFP) for Non-Personal Optometric Services for the Zuni-Ramah Navajo Indian Hospital, located in Zuni, New Mexico 87327. Contractor to provide Optometric Services including primary care, vision screenings, consultations and general therapeutic services for patients referred to the Optometry Clinic. The required services are for one (1) day per week for 52 weeks per year. Additional Days of services shall be mutually agreed upon between the Contractor and the Health Facility. Offeror must possess a current license to practice Optometry from a State of the United States or its territories. The contract shall be for a base year with four (4) one year options.
