The following additional terms apply: Any contract awarded as a result of proposal submitted under this RFQ shall extend from 1 Jan 2000 or date of contract award, whichever is later, through 31 Dec 2000, unless sooner terminated under the terms of the contract. Exercise of the Government's option(s) to extend this contract may extend the above 31 Dec 2000 date to as late as 31 Dec 2003. FAR Clause 52.217-9 (already included has fill in as follows: (a) 30 calendar days before contract expiration and (c) base period plus 3 option periods). This contract is subject to the provisions of the Service Contract Act of1965 (P.L. 89-286) and the requirements of the Service Contract Act as amended by P.L. 92-473, 9 October 1972. Attention is invited to the obligation of the Contractor under Section 4(c) of the amended Service Contract Act. The Contractor, in performing this contract, shall adhere to the provisions of said Act and the regulations of the Secretary of Labor. Any questions regarding the extent of these obligations should be addressed to the Department of Labor. The following U.S. Department of Labor Wage Determination is applicable to this procurement: 94-2525(Rev. 14), dated 8/31/1999. The Contractor shall obtain all necessary permits, give all notices, pay all license fees and taxes, comply with all Federal, state, municipal, county, and local Board of Health ordinances, rules and regulations applicable to the business carried on under this contract, and be responsible for all applicable state sales and use taxes. Any award made as a result of this solicitation will be for the base period only and will not include any option period. FAR Clause 52.219-23 (which is incorporated in FAR Clause 52.212-5-already applicable) applies. DFARS Clause 252.204-7004 applies. FAR Clause 52.232-18 is deleted. When submitting references on similar services provided to Government or commercial customers include point of contact and telephone number of POC. Offerors shall submit the certifications in FAR Provision 52.212-3 and DFARS Provision 252.212-7000 with their proposal.
