This is a combined synopsis/solicitation for commercial items prepared in accordance with (IAW) FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. II. Solicitation is issued for a Request for Proposal, (RFP)# F41652-99-R0048. Award will be made based on Performance Price Trade-Off. III. This solicitation document & incorporated provisions & clauses are those in effect through Federal Acquisition Circular 97-13. IV. This is small business set-aside procurement under SIC 7991 Physical Fitness Facilities ($5M). V. There are 5 contract line items numbers (CLINs). Workload Estimate for FY00 (1 Nov 99-30 Sep 00)=190 customers per day, 152 towels used daily, class & services registration $1000 per month/8 people per day, sundry sales $5 per day. Offerors are to submit written bid proposals as 5 separate CLINs, number 0001 as Base Year& numbers 1001 through 4001 as Options with Unit of Issue & Total Bid for each year. Bids are to be provided in the following format IAW the Performance Work Statement (PWS). CLIN 0001 IAW PWS, on or about 1 Nov 1999 or date of award whichever is later to 30 Sep 2000, (Base Year). Operation of Health Club $__ per month (unit price), Total Amount $__, (11 months). CLIN 1001 IAW PWS, 1 Oct 2000 to 30 Sep 2001(Option Year One) Operation of Health Club $__ per month (unit price), Total Amount $__, (12 months). CLIN 2001 IAW PWS, 1 Oct 2001 to 30 Sep 2002(Option Year Two) Operation of Health Club $__ per month (unit price), Total Amount $__, (12 months). CLIN 3001 IAW PWS, 1 Oct 2002 to 30 Sep 2003(Option Year Three) Operation of Health Club $__ per month (unit price), Total Amount $__, (12 months). CLIN 4001 IAW PWS, 1 Oct 2003 to 30 Sep 2004(Option Year Four) Operation of Health Club $__ per month (unit price), Total Amount $__, (12 months). Grand Total Base plus Four Option Years $__. Bids shall be received by 22 Oct 99. VI. DESCRIPTION: PWS: The contractor shall provide personnel to Operate the Dyess AFB Health Club Facility. Hours of operation are Monday -- Friday 6:00AM 6:00PM; Saturday, Sunday & Holiday 9:00AM 9:00PM. Facility is open year round except for Thanksgiving and 25 Dec. Contractor shall be responsible for cashier services on an as needed basis for sundry items sold, locker rental, class & service fees. AF Form 2557 will be used & excess funds turned in daily to the designated government representative. The government will provide operating funds & Sundry items, with contractor responsible for any discrepancies. Government will provide yearly training on cashier services to contractor personnel. Contractor shall on an hourly basis ensure sanitation, cleanliness, safety, & order within the facility to include the front exterior area to the curb. Services shall include but are not limited to cleaning of spills, flushing toilets, ensuring equipment is returned to designated locations, noting any interior facility damages & counting of personnel in building; Form 7 SVS/SVMP FL 2, Hourly Time & Attendance Record shall be used. Fitness equipment shall be wiped down on an hourly basis with an approved sanitizer (50ppm chlorine solution) furnished by the contractor. Contractor shall use AF Form 1297, Temporary Hand Receipt or fitness desk logbook for equipment that is issued. On an as needed basis the contractor shall demonstrate basic exercise & fitness techniques on equipment. Contractor shall perform minor adjustments & repairs on equipment. Equipment, which can not be repaired, shall be reported immediately to the Fitness Center Director for disposition. Contractor shall provide & restock both lockrooms' shower soap in government provided containers. Soap shall be Dial brand deodorant soap #3829011 or approved equal. Contractor shall issue & maintain government furnished towels, including but not limited to cleaning, drying and folding within each 12 hour shift. Government will provide bleach & laundry detergent as required. Washer & dryer will provided by another government contract. SECURITY: Contractor shall be responsible for facility security at all times. CONTRACTOR PERSONNEL shall have current valid professional certifications in cardiopulmonary resuscitation (CPR) and First Aid. Certifications to be provided to Contracting Officer. GOVERNMENT FURNISHED PROPERTY & MATERIALS: 190 towels, $50.00 cash, bleach, laundry soap, contract washer & dryer, miscellaneous sundry items, 72 pieces of fitness equipment, 24 issue items, 28 mats, 1 cash register and 26 miscellaneous desk items. Joint inventory will be done prior to contract start date. DRESS & APPEARANCE: Contractor personnel dress & appearance shall be professional and easily recognized as contract employees with contractor provided name tags. All contract employees will be required to attend annual Customer Service training provided by the government. Contractor shall be in compliance with Air Force Manual 34-137, Titled "Air Force Fitness and Sports Operation" paragraphs 8.5, 8.8, 8.11, & paragraph 8.4 -- 8.4.3.6. http://www.dyess.af.mil/pdo/ VII. Required date if performance begins 01 Nov 99 or date of award whichever is later. Place of performance is Building 7007, 617 3rd St, Health Club, Dyess AFB, TX. VIII. Solicitation provision at FAR 52.212-1, Instructions to Offerors Commercial Items applies to this acquisition and any addendum to the provision is hereby incorporated by reference. IX. 52.212-2-Evaluation-Commercial Items(a)The basis for award will be an integrated assessment based on tradeoff between past performance and price. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: For the purposes of award, offers will be evaluated based on the following factors, listed in descending order of importance: Factor 1.Past Performance. Sub-factor a-Quality of Product/Service, Sub-factor b-Management of Key Personnel, Sub-factor c-Schedules, Sub-factor d-Business Relations. Each proposal will be evaluated and given a Confidence assessment rating. The rating will be assessed for each factor as follows: Unsatisfactory/No Confidence (1) Based on the offeror's past performance, extreme doubt exits that the offeror will successfully perform the required effort. Marginal /Little Confidence (2) Based on the offeror's past performance, substantial doubt exits that the offeror will successfully perform the required effort. Changes to the offeror's existing processes may be necessary in order to achieve contract requirements. Satisfactory/Confidence (3) Based on the offeror's past performance, some doubt exits that the offeror will successfully perform the required effort. Very Good/Significant Confidence (4) Based on the offeror's past performance, little doubt exits that the offeror will successfully perform the required effort. Outstanding/High Confidence (5) Based on the offeror's past performance, essentially no doubt exits that the offeror will successfully perform the required effort. Neutral/None No performance record identifiable (see FAR 15.305 (a)(2)(iii)) & (iv). No more than 15 references are to be provided by offerors and investigated for past performance from the past 3 yrs. Any derogatory past performance information obtained will be presented to the affected offeror & that offeror will have an opportunity to respond with any mitigating facts. The final determination of past performance ratings shall rest with the Contracting Officer. Factor 2.Price. Past performance shall be more important than price. (b)Options. The Government will evaluate offers for award purposes by adding the total price for Options 1 thru 4 to the Total Price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.
