This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quotations are being requested and a written solicitation will not be issued. The Solicitation Number is MDA410-00-R-0001. This action will be processed using simplified acquisition procedures. The Department of Defense Education Activity (DoDEA) is an education activity and qualifies for academic/educational pricing. DoDEA requires expert assistance in the development of the annual school accountability report which, will summarize test, demographic, survey and other data. The data will include graphic representations and narrative descriptions of DoDEA efforts to improve educational and organizational accountability. The report will be made available to DoDEA constituents, including the U.S. Congress, military departments, and state education agencies. GENERAL REQUIREMENTS: Approximately 44 pages (back to back), up to 4 colors with approximately 35 color photographs, matte-coated white text stock (80lb.), laminated cover stock, text, tables, graphs and clip art. Basic design, layout and document format, raw data and text to be provided by DoDEA. Tables, graphs, artwork and production of draft report are to be developed by the contractor in close cooperation with DoDEA. The contractor will produce, using DoDEA data, cover design, report design, and layout, and document format for the report which, may include insertion of photographic images or incorporation of DoDEA student artwork. The contractor will provide several mock-ups and a maximum of 2 blue lines for review by DoDEA. The contractor will accept from DoDEA, raw data for systemwide education and management programs and demographic characteristics and will present data in the design, layout and production of reports associated with each category of data. The contractor will develop suitable graphic representations of SY98-99 results of the DoDEA evaluations, Comprehensive Test of Basic Skills (CTBW) and other test data, and demographic information for inclusion in the report. The contractor will provide 6,000 printed, color, bound (saddle stitched) reports and 1 (one) camera-ready negative. The contractor will deliver the initial page layout of report (presentation of data) 20 days after award; report format (type of publication) 30 days after award; and delivery of final product by mid December 1999. The contractor will prepare for shipment and delivery by Federal Express approximately 200 packages containing 10-25 copies each of the final publication to be delivered to area and district offices, schools, and advisory groups using counts and addresses provided by DoDEA. Each package will include a letter from the Director and copies of the evaluation forms to be provided by DoDEA. The packages prepared for Federal Express shipment and the remaining copies of the final publication will be delivered to DoDEA. The following clauses/provisions apply to this solicitation and are in effect through FAC 97-14 and DCN 19991001 Edition: FAR 52.212-1, Instructions to Offerors-Commercial Items; 52.212-2, Evaluation-Commercial Items. A single award will be made to the responsible offeror whose offer conforms will all requirements of this solicitation and whose proposal reflects the best value to the Government. Proposals will be evaluated based on capability, quality of product, and price. 52.212-3, Offerors Representations and Cetifications-Commercial Items, which MUST accompany the offeror's bid/quote; 52.212-4, Contract Terms and Conditions-Commercial Items; 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Comercial Items; 52.222-26, Equal Opportunity; 52.222-35, Affirmative Action for Special Disabled and Viet Nam Era Veterans; 52.222-36, Affirmative Action for Handicapped Workers; 52.222-37, Employment Reports on Special Disabled Veterans and Veterans of the Viet Nam Era. The following DFAR provision is hereby incorporated by reference: 252.204-7004, Required Central Contractor Registration (CCR). The SIC code is 2732. CLIN 0001 -- Initial page layout of report; CLIN 0002 -- Develop report format; and CLIN 0003 -- Develop and deliver 6000 printed color reports and 1 (one) camera-ready negative. The following items shall be sumitted in writing, to the address shown above, by 12:00 p.m. EST, 26 Oct 99: 1. Proposal, along with Representations and Certifications; 2. Product sample (sample of similar work not to exceed 25 pages). Samples will not be returned; 3. Two references (point of contact name and telephone number).
