Contract Specialist, 606-253-8892 The Federal Bureau of Prisons intends
to award a non-personal service contract tothe qualified
individual/business to provide optometry services for the Federal
Correctional Institution Elkton, OH. This procurement is 100% set aside
for small business. Duties include but are not limited to: evaluation
of visual acuity of patient, determine need for corrective lenses,
determine need for follow up and frequency of follow up examinations,
evaluate patients for need fo referral to Opthalmologist, evaluate for
glaucoma, determine level of blindness, determine specific medical
need for contact lenses, determine specific medical needs for
sunglasses, supply prescription for corrective lenses, consult with
staff physician/medical director on further or immediate treatment
needed on a patient, evaluate eye injuries, treat or recommend
treatment through referral, evaluate level of malformations/eye
deformities, document evaluation/treatment/recommendations/referrals on
consult form, provide continued medical education/training to medical
staff on injuries and needs for immediate referrals, and make
recommendations for medications needed on patients to the staff
physician/medical doctor for prescriptions as needed with appropriate
approval. Requirements include: the contractor must be a
licensed/certified optometrist in the state of Ohio. They must be a
graduate of an approved school of optometry. The contractor will
conduct approximately 6 sessions per month. Each session is defined as
three hours in length. Sessions will be scheduled between the hours of
7:30 am and 4:00 pm Monday through Friday, (excluding federal
holidays). The schedule shall be coordinated with the Contracting
Officer Technical Representative. Total work hours shall not exceed 30
hours per week. This will be a non-personal service contract and not
a personnel appointment. The contract period shall include a base year,
not to exceed 12 months, and four one year options. The anticipated
solicitation issue date is October 21, 1999, with request forquote due
