V -- TRANSPORTATION: AMBULANCE SERVICES SOL RFP 618-54-00 DUE 110299 POC Mavis Nelson 612-725-2189 Provide fully licensed, vehicle-equipped and staffed Ambulance Services to transport scheduled (non-emergent) VA Medical Center beneficiaries as directed for Department of Veterans Affairs Chippewa Valley VA Clinic, 2503 County Road I, Chippewa Falls, WI; offeror must have a base of operations at which the physical plant houses ambulances, related equipment and personnel; provide appropriate personnel, vehicles, and equipment to provide a level of care so as to ensure that life-threatening situations and potentially serious injuries can be recognized for Basic Life, Advanced Life and Critical Care Advance Life ground ambulance services . Line Item quantities are only estimates and the Government does not guarantee any number of trips or mileages. Pricing will include incidental and miscellaneous costs associated with any transport at a Base Rate to include the first 10 loaded miles and a 15 minute wait time.
