clinics during contract period (November 9, 1999 through October 31,
2000), with 44 scheduled patients to be held in the Community Health
Department at the Cherokee Indian Hospital. The clinics shall last
approximately 8 hours per day(8:00 AM -- 4:30 PM). Any rescheduling of
clinic days or times shall involve consultation with the Contractor by
the Government, but shall be at the convenience of the Government. 2.)
Services required shall include: refractive errors, Prescribe
corrective leneses/contact lenses, perform tension checks, Screen for
other disease processes, Performa post-op checks and treatment
follow-ups, consult with staff physicians regarding referrals and
findings of patients requiring treatment or surgery, and Provide an
assistant for ech on-site clinic to screen vision, dilate eyes, check
pressures and blood sugars. 3.) The following equipment will be
provided by the Government at no additional cost or charge to the
Contractor in conducting on-site clinics: refractor, slit lamp,
tonometer (manual and air puff), black wand occluder, reading cards,
adult trial frames, opthalmoscope, and projector. 4.) The following
equipment is to be supplied by the Contractor at no additional cost to
the Government for the clinics: Prisms, keeler clips, pediatric trail
frames, and indirect opthalmoscope. 5.) Reports of findings will be
required on each patient seen. these will be filed in the patient and
CHN files and given to the rehabilitation counselor (if needed) and the
hospital physician as necessary. Documentation will be done on Cherokee
Indian Hospital forms. 6.) The Contractor shall provide services to
In-patients and Out-patients scheduled by the Government physicians at
the Cherokee Indian Hospital. The Contractor shall take a history and
perform an appropriate examination. the findings shall be correlated
with the laboratory and x-ray findings. The Contractor shall make a
verbal report to the Government physician and shall make a written
consultation on the patient's medical chart. The written consultation
shall include the findings, impression and recommendations about the
management. 7.) The contractor shall be available for consultation by
telephone and shall give recommendations to the Government physician
about management of patients seen between clinics at no charge to the
Government. 8.) The Contractor shall have: Certification by the
American Board of Opthamology, Unrestricted license to practice
medicine in North Carolina, Unrestricted privileges in the specialty of
opthamology at the hospital in which the major portion of the services
are rendered. It is required that no serious ethical charges are
outstanding and a review of previous ethical behavior shall be
convincing evidence that the contractor practices his/her specialty
