The contractor shall provide reference laboratory testing as required to the Cherokee Indian Hospital laboratory and the Snowbird clinic. The contractor shall perform non-emergency laboratory testing procedures listed on the attached schedule, and any other laboratory testing required, when ordered by the Cherokee Indian Hospital laboratory in Cherokee, N.C. or the Snowbird Clinic, in Robbinsville, N.C. Each facility shall have the perogative to direct emergency testing (as defined by the clinician) to any laboratory of its choice for expedient results reporting. The contractor shall provide in-house, the majority (90% or more) of testing received from the IHS facilities. The Contractor shall include with the bid and after award of the contract provide to each IHS facility laboratory: a list/catalog of tests/procedures performed in the contractor's laboratory where specimens are sent, to include instructions for specimen requirements and handling; a contractor's laboratory testing frequency with a turnaround time schedule listing the expected time interval from receipt of specimen to reporting of test results, which will be met ninety percent (90%) of the time; there shall not be a separate charge for speciment handling/processing by the contractor's laboratory for those tests sent to a second laboratory or be invoiced as "miscellaneous tests". The contractor shall provide at no additional cost, to each requesting IHS laboratory, contractor's test requisition forms which shall be either: pre-printed, include the name of the facility from which specimens are sent, include the address of the facility from which specimens are sent, include the contractor's name and address, include a unique identification number of the facility from which specimens are sent, be in duplicate or more, be designed with a check-off system to list the commonly ordered tests on one page, or provides a computer at no additional charge that prints the requests and provides trackability, and entry into the data base of the laboratory test menu that supplies all of the above, in addition to clinical utility of each test and methods of each test (this method is preferred).
