commercial items prepared in accordance with the format in Subpart
12.6, as supplemented with additional information included in this
notice. This announcement constitutes the only solicitation; proposals
are being requested and a written solicitation will not be issued.
Solicitation 626-99-41-jo is issued as a Request for Quote (RFQ). This
solicitation document and incorporated provisions and clauses are
those in effect through Federal Acquisition Circular 97-13: FAR
52.212-1 Instructions to Offerors; FAR 52.212-4 Contract Terms and
Conditions Commercial Items; FAR 52.212-5, Contract Terms and
conditions Required to Implement Statutes or Executive Orders- This
acquisition is unrestricted. SIC is 3841 and small business size
standard is 500 employees. This requirement is for an Intravascular
Ultrasound (IVUS) System for use in our Cardiac Cath Laboratory. This
system must allow interventionalists to obtain unique and additional
information about the vessel wall compared to angiography. It is to
provide definite enhanced diagnostic capabilities, information about
lesion location, percent stenosis, lesion length and lesion morphology.
It must include a Real Time Tissue Acquisition Board that does not use
phased array averaging techniques, resulting in more accurate vessel
display, and a High Contrast digital Graphics Board that clearly
displays calcification, soft plaque, and other plaque morphologies. It
must have Automatic Catheter recognition that will instantly set the
system controls for optimal imaging.. It must have an upgradeable
platform for future upgrades. The system will, at a minimum, include a
monitor, automatic pull back device, computer key console, printer
(Panasonic model AG-MD830 or equivalent), paper, motordrive for
catheters, foot pedal, demo probe, VCR (Sony Model UP-890MD or
equivalent), video tape instructional book and a data/service that
includes a parts list and schematics. Minimum of one year warranty on
all hardware. Delivery is to be made to the Chief, A&MMS, VA Medical
Center, 1310 24th Avenue South, Nashville, TN, within 60 days after
receipt of order. Acceptance and Inspection at VA Medical Center,
Nashville, TN. FOB Destination. Companies wishing to respond to this
solicitation should provide this office with the following: (1) A price
quote on company letterhead for the requested item showing solicitation
number, unit price, extended price, FOB, prompt payment terms and
correct remittance address along with a completed copy of FAR Clause
52.212-3, Offeror Representations and Certifications-Commercial Items.
( Those companies not in possession of the clause in full text may
contact this office and a copy will be faxed to you); (2) In addition
the bidder shall furnish as part of its bid all descriptive literature
and technical data sheets necessary for the purchasing activity to
determine whether the product offered meets the minimum salient
characteristics/requirements of the solicitation. Failure to provide
this information will cause your bid to be rejected as nonresponsive.
Award of Best Value for the government contract will be based on
ability to meets the requirements specified above, evaluation by end
user, pricing, delivery period, and warranty. Offers are due by close
