Kendrick, Senior Contracting Officer, (540) 983-1048 The VA Medical Center, Salem, Virginia has a requirement to utilize a pre-existing clinic in the Tazewell, Virginia area, complete with primary care physicians and personnel required for routine primary care services. At the time of response to this solicitation, the clinic must be in compliance with JCAHO requirements. The clinic must have either on-site lab, radiology and pharmaceutical capability OR be able to demonstrate that a contractual relationship exists to provide off-site lab, x-ray, and pharmaceutical services to the existing clinic. Clinic must be located with a twenty (20) mile radius of the city of Tazewell, Virginia and be located within the VA Medical Center's primary service area. Physicians, nursing staff, and technical staff providing services under the resultant contract must meet all evaluation criteria and other professional certifications and licensing requirements. The qualifications of such personnel shall also be subject to review and approval of the Chief of Staff of this Medical Center. The VAMC, Salem intends to negotiate a capitated rate per patient. It is anticipated 350 veterans from the counties of Bland, Tazewell, Buchanan, Smythe, Wythe, Russel and Giles will utilize this clinic. Clinic profiles will be established as clinic staff deems appropriate to accommodate workload demand. This acquisition is unrestricted and SIC 8099 applies.
