The Portland VA Medical Center requires commercially available Hospice Services for patients referred by the Department of Veterans Affairs in accordance with all the terms and conditions of the contract. Services will be performed in the local Portland, Oregon and Vancouver, Washington areas and surrounding counties. Offerors must be accredited by the Joint Commission on Accreditations of Healthcare Organizations (JCAHO) according to Hospice standards, Medicare Certified and licensed by the state(s) in which service will be provided. Services provided will include the following as defined in the solicitation: Registered Nurse, Social Worker/counselors, Home Health Aids, Volunteers, Physical Therapy, Occupational Therapy, Speech Pathology, Chaplain Services, Dietary Consults, Family Counseling, Bereavement Care and Counseling, Medications relating to terminal illness, Routine Medical Supplies, Durable Medical Equipment (excluding ventilators), In-home lab fees, Continuous Care (when required), Respite Care, Palliative Therapies (radiation or chemotherapy), Ambulance Transportation, and Inpatient care as arranged between the Hospice and PVAMC. This contract will delineate reimbursement rates when home health services are paid directly by PVAMC. Services shall be provided for the period beginning January 1, 2000 through December 31, 2000 with two (2) options to renew for a period of one (1) year each.
