combined synopsis/solicitation for commercial items prepared in
accordance with the format in Subpart 12.6, as supplemented with
additional information included in this notice. This CBD announcement
constitutes the only solicitation; proposals are being requested and a
written solicitation will not be issued. Request for Quotation (RFQ)
618-59-00 entitled "MRI Magnet Maintenance/Cryogen Service" and
incorporated provisions and clauses are those in effect through Federal
Acquisition Circular #14. The SIC Code for this procurement is 7389 and
the small business size is $5.0 million. Contractor's proposals are
hereby being requested in response to requirements for brand name or
equal products listed below and shall be submitted on contractor's
letterhead or bid form and must address all requirements listed and
must contain the name, title, and signature of person authorized to
submit proposals/bids on behalf of the contractor. This is a
requirements contract for the remainder of Fiscal Year 2000, with one
12-month option to renew for Fiscal Year 2001. Price proposal will
include separate price schedules for each Fiscal Year. Item One:
Provide parts/service contract on GE Signa Magnetic Imager, SN: 64320
(not including MMCRYOS). MRI Maintenance contract to include: a) VA
Medical Center Biomedical Instrumentation Service personnel will
provide regular scheduled PM's and first response for repairs; b)MRI
Maintenance contract will provide all parts for either the PM's or
repairs; c)MRI maintenance to include complete PM and repair support
for the GE MRI Signa Advantage System if necessary; d)MRI repairs
response time will be no more than four hours by phone, 24 hours
on-site; e)Vendor must have capability to interface the MRI system to
a remote diagnostic site reducing on-site repair times. Item 2: Provide
MRI Magnet Maintenance and cryogen services (MRI Signa system, Magnet
type:S1, Platform: Signa Advantage 4.8). Contract to include the
following: a)provide all cryogens, to G.E. specifications, necessary to
maintain magnet operations; b)Provide all MRI compatible containers
(non-magnetic dewar), transfer lines, and tools to perform cryogen
fills; c)Monitor cryogen levels, coordinate delivery of cryogens and
the fill schedule with VA personnel; d)Perform cryostat vacuum
repumping at necessary intervals to maintain the magnet's cryostat
vacuum within the VAMC's equipment specifications; e)Cryogen fills to
be performed after normal business hours, typically this will be
between the hours of 5:00p.m. to 5:00a.m. Monday through Saturday;
f)Contractor is responsible for any damage to the damage to the magnet
or its components that may result from cryogen fills, all repairs to
be done to G.E. specifications (includes any damage to VA property);
g)Demonstrated ability to fulfill a cryogen service agreement on a G.E.
Signa magnet. All supplies provided will be Y2K compliant. All
responsible sources who can meet the VAMC's requirements and provide
services/items as listed above may respond to this solicitation by
submitting a price and technical proposal on their company letterhead
or bid form. Evaluation factors will include, but may not be limited
to, technical ability of company to meet Governments requirements, past
performance and price/price related factors. The solicitation clauses
incorporated herein are those in effect through FAC 97-14 and are
