The Contractor shall provide on-site computerized tomographic (CT) radiologic studies and interpretations for various Navajo Area Indian Health Service hospitals and health centers. Contractor shall provide CT scan studies in a Government-owned facility at Tuba City Indian Medical Center, Tuba City, AZ. The Contractor shall provide all necessary personnel, medical equipment, and supplies required to deliver CT scan services. The Contractor's medical personnel shall be licensed, certified, and/or registered with the appropriate Boards, Registry, and/or State of Arizona. Resultant contract will be a requirement-type contract to be effective for an initial twelve (12) month period with two (2) one-year option periods. All requests for solicitation must be in writing.
