Q -- RADIOLOGY INTERPRETATION SERVICES SOL RFQ 452-0002-0 DUE 120599 POC Kathleen Stone- 1-352-330-3017 The Bureau of Prisons, Federal Correctional Complex, Coleman, Florida, is soliciting quotes on a full and open competitive basis from qualified individuals and organizations for Diagnostic Radiological Interpretation services. This non-personal medical services contract is to provide interpretations of all standard x-rays perforemed at the Federal Correctional Complex. Services will be provided at the Contractor's place of business. The contractor will provide results via facsimile, within 48 hours, with typewritten results on submitted x-ray requests within 96 hours. Contractor will provide shipping of x-ray films (2 day delivery from place of business) to and from (4 day delivery from place of business to FCC Coleman) the contractor's location. The contract period will include a base year (date of award through September 30, 2000) with four (4) one year option periods for renewal. The estimated issue date of the Request for Quotations 452-0002-0 will be November 12, 1999, with an anticipated due dateof December 7, 1999. Services will be provided on a daily basis, Monday through Friday. The estimated number of x-rays for the base year is 1000, with increases for each option year. Special qualifications include, proof of license to practice medicine, Board Certification in Radiology, proof of medical insurance, and credential verification documents to include three (3) references.
