DADA13-99-T-0007 for Urgent Care Physician Services is a combined
synopsis/solicitation for commercial items prepared in accordance with
the format in FAR Subpart 12.6, as supplemented with additional
information included in this notice. This announcement constitutes the
only solicitation; proposals are being requested, and a written
solicitation will not be issued. This procurement is being conducted
under the Subpart 13.5 test program, simplified acquisition procedures
apply, and conducted with a combination of Parts 12 and 15. CONTRACT
REQUIREMENTS: Provide qualified physician(s) with an M.D. or D.O.
designation and completion of an accredited Medical, surgical, or
primary care internship for the Urgent Care Clinic at Bassett Army
Community Hospital, Ft. Wainwright Alaska QUALIFICATIONS: Physicians
shall have trained in the practice of medicine, have completed a
clinical training program in a primary care specialty, and/or have
current experience in full spectrum primary care to include experience
with adults, pediatrics and women's health. Waivers for portions of
these requirements may be issued at the discretion of the Commander.
Physicians shall have and maintain a valid, unrestricted license to
practice medicine in the U.S. and have and maintain a current DEA
registration number. Physicians shall have and maintain current
certification in Advanced Cardiac Life Support (ACLS), Basic Life
Support (BLS) and Pediatric Advanced Life Support (PALS). SCHEDULE: The
UCC at Bassett is primarily a walk-in clinic providing full primary
care for acute episodic and urgent care for adult male, female and
pediatrics patients. Approximately 50% of the patients presenting to
the UCC are pediatric patients. Although primarily a walk-in clinic, a
maximum of 3 patient appointments per hour may be scheduled.
Physicians shall work a minimum of 336 hours per month in rotating 12
hour shifts with an additional option of 12 overtime hours per month.
Services required are as follows: CLIN 0001 -- 1 FTE to begin
approximately 1 December 1999 through 31 May 2000 (1008 hours @ $______
per hour); CLIN 0002 -- 1 FTE to begin approximately 1 February 2000
through 31 May 2000 (672 hours @ $_____ per hour); CLIN 0003 --
Overtime Hours for the period 1 December 1999 through 31 May 2000 (35
hours @ $_____ per hour). Option periods may also be required, as
follows: CLIN 0004 -- 2 FTE's to begin 1 June 2000 through 30 November
2000 (2016 hours @ $_____ per hour) and CLIN 0005 -- Overtime Hours
for the same period (35 hours @ $____ per hour). Overtime hours are
approximate only. It is preferred that these services be provided by a
no more than 3 different physicians to ensure continuity of services
however, it is recognized that, due to the location and short lead
time, this may be difficult. Therefore, the contractor is authorized to
use short term physicians to provide these services however, no more
than 10 physicians will be credentialed in the course of this contract
and its options, if exercised. CREDENTIALING: No physician shall
provide services until they have been credentialed by the hospital.
Credentialing specifics will be provided. SPECIFIC TASKS: To include,
but not limited to: examination, assessment, diagnosis, treatment and
follow-up of general medical conditions, primary medical care including
prevention and health education and counseling, routine school
physicals, sports physicals and other physicals and consultations.
Respond to staff nurse requests for assistance in cases of in-hospital
emergencies. ADMINISTRATIVE: Incorporated provisions and clauses are
those in effect through Federal Acquisition Circular 97-14. SIC is
8011; small business size standard is $5 million dollars. CLAUSES AND
PROVISIONS: The following clauses and provisions apply to this
acquisition: FAR 52.212-1, Instructions to Offerors Commercial Items;
52.212-3, Offeror Representations and Certifications Commercial Items
(offeror is required to submit a copy of this provision, completing up
through paragraph (c)3), 52.212-4, 52.212-5 (specifically 52.222-26,
Equal Opportunity; 52.222-35, Affirmative Action for Disabled Veterans
and Veterans of the Vietnam Era; 52.222-36, Affirmative Action for
Workers with Disabilities; and 52.232-33, Payment by Electronic Funds
Transfer Central Contractor Registration) 52.232-3, Payments under
Personal Services Contracts; 52.249-12, Termination (Personal
Services); 52.217-9 Option to Extend the Term of the Contract ((a)
notice to the contractor at least 30 calendar days prior to expiration,
(c) 1 year; 52.212-2 Evaluation Commercial Items. Offers will be
evaluated based on technical capability, past performance and price.
Technical capability includes the following: 1-Current state license;
2-Quality, length and sufficiency of education and experience;
3-Ability to meet privileging standards of the US Army; 4-Possession of
the necessary certifications; 5-Date available to begin service. The
closing date for receipt of offers is November 15, 1999. Offerors are
