The Veterans Affairs Healthcare Network Upstate New York (VAHNUNY) desires to continue a contract for Community Based Primary/Preventative Care services for veterans in this geographic region which has been served by this mechanism since 1993. The resulting contract will be for a period of one year with two options to commence on or about April 1, 2000. The Contractor will be responsible to furnish medical facilities, clinical providers, equipment, supplies, routine radiology and administrative functions to fulfill the level of care specified in the RFP.
