This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only Request for Quotation (RFQ); quotation is being requested and a written RFQ will not be issued. This RFQ, number 00L005622, is for the annual Coho software database subscription and maintenance renewal of order 98L00561001D. The Contractor shall provide a quantity of one (1) each of Coho software data upgrades and maintenance v.2.2 for the period of one (1) year. The Department of Health and Human Services (HHS), Program Support Center, knows of only one source, Avue Technologies Corporation, 1142 Broadway Plaza, Suite 400, Tacoma, WA 98402. Request for Quotation documents, provisions and clauses herein are those in effect through Federal Acquisition Circular 97-13. The Standard Industrial Classification code is 5045 with a small business size standard of 100.
