The Veterans Affairs Medical Center, San Francisco, CA., requires courier services for laboratory specimens from Ukiah (94577), and Santa Rosa (95404) VA Outpatient Clinics to Laboratory Medicine Services at the VAMC San Francisco (94121). The courier services shall be Monday through Friday (except Federal Holidays), leaving Ukiah at 15:30 -- 15:45 and Santa Rosa at 16:30 -- 16:45) to arrive at VAMC San Francisco Lab no later than 19:00. Any additional runs shall be charged at the same rate. Vendor ensures that drivers are trained in handling and transportation of laboratory specimens and that specimens are transported in an acceptable manner to preserve integrity of sample and protect driver from contamination. Vendor shall provide VA with a copy of their PUC license and their insurance binder. Vendor will bill monthly and invoices will be paid in arrears from obligations set up on 1358's. Invoices must be mailed directly to Santa Rosa Clinic (address/POC to be provided at time of award). Periodof Performance is nine (9) months, starting January 2000 through September 30, 2000.
