1999, TO INCLUDE AN ADDITIONAL SITE VISIT PLANNED ON DECEMBER 20, 1999.
PARTIES INTERESTED IN OBSERVING THE WORK SITE SHOULD MEET NO LATER THAN
10:00 A.M. AT THE CENTRAL UTAH WATER CONSERVANCY DISTRICT OFFICE, 766
N. CENTER ST., DUCHESNE, UTAH. POINT OF CONTACT IS KENT KOFFORD (801)
379-1151. The work involves sealing three transverse cracks through
Upper Stillwater Dam, a roller compacted concrete (RCC) dam completed
in 1987. The transverse cracks to be sealed are located at approximate
stations 25+20, 41+10, and 42+85 and occur over the full height and
thickness of the dam. Sealing of the cracks shall consist of a
contractor-designed upstream PVC membrane system that will prevent
leakage through the selected cracks. The work involves both below water
and above water work. This is a negotiated competitive, firm-fixed
price requirement. Technical and cost or price proposals will be
required. Technical factors to be evaluated will be the offeror's
experience, design, installation procedures, and safety record. Cost or
price factors to be evaluated will be the total price, and realism and
trackability of the proposed costs. A subcontracting plan will also be
required to be submitted with proposals. The work is located at Upper
Stillwater Dam approximately 44 miles northwest of Duchesne, Utah in
Duchesne County. Estimated price range of job falls in the step between
$1,000,000 to $5,000,000. Bid, performance, and payment bonds will be
required. Work is under SIC Code 1629. Contract performance is to be
completed not later than December 1, 2000. All responsible sources may
submit proposals which will be considered for award. A site visit is
planned on November 30, 1999. Parties interested in observing the work
site should meet no later than 11:00 a.m. at Upper Stillwater Dam,
Utah. Signs will be posted at site. Point of contact for site visit is
