The Department of Veterans Affairs Medical Center, Birmingham, Alabama intends to negotiate with Olympus America Inc. for Cost Per Test Endoscope procedures at an estimated 3,000 procedures per year. The following are the requirements that the system must be able to accomplish and the required equipment to accomplish the Cost Per Test. (1) The system must be equipped with two 2 stretcher chairs which, feature easy stair to stretcher conversion, adjustable height, reverse Trendelenburg position capability, C-arm compatibility, an integrated back rest positioning, rotatable side arms, backrest release mechanism three 3 section 2 " thick mattress four 4 six inch anti-static casters, three position brake mechanism and four position IV pole and other features associated with Olympus part number 55490. (2) An Ultra Slim Video Pediatric Gastroscope with high resolution, high color definition images for a wide range of applications, 8.7mm distal end outer diameter, 8.5mm insertion tube outer diameter, 2.2mm instrument channel, 120-degree field of view, 1030cm working length and angulation of 210/90-degrees (up/down), 100/100-degrees (right/left), and other features associated with part number 10062 GIF-P140. (3) A pediatric Video Colonoscope with 11.3 insertion tube diameter. 3.2mm channel diameter, 140-degree field of view, 1,680mm working length, angulation 180/180-degrees (up/down), 160/160-degrees (right/left), and all other features of part number 10085 PCF-140L. (4) Videoprocessor with microprocessor for physician switch per-set, pre-entry of patient data, remote switching for documentation and subscreen imaging. Requires CLV-U40, U20 light source, and other standard features on part 30010 CV-140. (5) OES Xenon Light Source (300W) with flash for still photography, Polaroid, cine, and video with emergency 150W lamp, part number 32725 CLV-U40. (6) 19" color video monitor, part number 43517B OEV-203. (7) Four Endoscopic Procedure Carts with 3 shelves including a top shelf designed for CV-100 and CLV-U20 placement, three drawer (one lockable), a 10-unit forceps rack, multiple outlet plug strip and 5"anti-static casters, and other standard features of part number 55374 TK-E2. (8) Four Imaging Carts that will accommodate a 19 or 20" monitor, part number 55388 TI-1900. One of the endoscopic procedure carts and monitors should be installed in a single cart. (9) Four Video Sigmoidoscopes with 13.3mm diameter, 3.7mm 140-degree field of view, 73cm working length and angulation of 180/180-degrees (up/down), 160/160-degrees (right/left) and all features associated with part number 10082 CF-140S. (10) Five Video Gastroscopes with wide screen imaging, with 10.5mm diameter, 2.8mm channel, 140-degree field of view, 103cm working length, and angulation of 210/90-degrees (up/down), 100/100-degrees (right/left), with standard features and accessories of part 10061 GIF-Q140. (11) One Therapeutic Video Gastroscope with wide screen imaging, with 12.5mm diameter, double channel, one of 4.2 mm, 140-degree field of view, 103cm working length, and angulation of 210/90-degrees (up/down), 100/100-degrees (right/left), with standard features and accessories. (12) Five Video Colonoscopes with wide screen image, 12.9mm diameter, 3.7mm channel, 140-degree field of view, 168cm working length and angulation of 180/180-degrees (up/down), 160/160-degrees (right/left), and standard features and accessories of part 10083 CF-Q140L. (13) Large Channel Video Duodenoscope with 12.5mm insertion tube diameter, 4.2mm channel diameter, 100-degree field of view (side viewing 5-degree retro), 1,240cm working length, angulation of 120/90-degrees (up/down), 110/90-degrees (right/left), with fixed cap, part number 10073 TFJ-140F. (14) Video Duodenscope with 11.5mm insertion tube, 3.2mm channel, 124cm working length, 100-degree field of view, angulation 120/90-degrees (up/down), 110/90-degrees (right/left), 15-degree retrograde, with fixed cap, part number 10072 JF-140F. In-house loaners of the above mentioned endoscopes: three colonoscopes, three gastroscopes, one duodenoscope and three sigmoidoscopes. (15) The following is a list of equipment to be used with this system in order to obtain the information needed by the physician: Part number 50102, EndoWorks Network Nodes, includes Pentium II CPU operating at 350 MHz with 64 Megabytes Ram, with VIGAT board, Ethernet 10-Base-T Network Interface Card, GI/Pulmonary ImageManager for Windows Software, Monitor, and Keyboard; Part 50106, Imagemanager for Windows Dictation/Review station; Part 50103J, Print Archive Server for Jukebox includes Pentium 350 MHz CPU, 64 MB RAM, 1.2 GB Hard Drive, Ethernet 10-Base-T Network Interface card, software, Modem, 8 GB Tape backup system, monitor, mouse, and keyboard; Part 50126, Endoworks fax server for dictated faxing; Part 50110C, 10 User EndoWorks Server for combined G.I./Pulmonary, Includes all Windows, and Applications software and hardware components including network software, tape backup and interruptible power supply; Part 52002, 2-User CHARTnote/GI Procedure Report Module to permit simultaneous reports generation by up to 2 users on any Network Node Workstation; Part 51021, Network Hub (12 Workstations); Part 56580K, Xerox Docuprint C55MP with 8mb Memory Upgrade; Part 58300 HL7 Gateway Server; Part 58300A, Olympus HL7 One-way Link; Part 51050A, Endoworks single drive 52 GB 20 Disk Jukebox, and all cables and hook-ups to ensure that all equipment performs as needed. The Olympus part numbers supplied in this announcement are for uses as a guide for any other offeror purporting to be able to furnish this system. Offeror may submit a written response along with cut sheets on all equipment that is being portrayed as equal to the listed equipment for review by the using service by December 14, 1999.
