synopsis/solicitation for commercial items prepared in accordance with
the subparts in Federal Acquisition Regulation 12.6, as supplemented
with additional information included in this notice. This announcement
constitutes the only solicitation; bids are being requested and a
written solicitation will not be issued. Solicitation number Request
For Proposal (RFQ) 673-42-00 is assigned and shall be referenced on any
proposals. This solicitation document and incorporated provisions and
clauses are those in effect through Federal Acquisition Circular 97-13.
The Standard Industrial Classification Code is 8049 and the small
business size standard is 5 million dollars. The James A. Haley
Veterans Hospital, located at 13000 Bruce B. Downs Blvd, Tampa, Florida
33612 has a requirement for Ultrasound type services at it's Brevard
Outpatient Clinic located at 2900 Veterans' Way, Melbourne, Florida
32940. Description of duties is as follows: Provide one registered
certified medical sonographer, registered in the following areas:
ultrasound (R.D.M.S.) Vascular (R.V.D.) Echocardiography (R.D.C.S.).
Qualified in more than one specialty and fully conversant with the
following equipment: ATL HDI 5000 ultrasound system, Hewlett packard
sonos 2500 system imexlab 9000 system and their components. Located at:
Brevard OPC. Period of this contract shall be from time of award until
September 30, 2000. It shall have four option years from October 1
September 30 in each of the subsequent years. Contract will terminate
on September 30, 2004. Offerors are required to submit pricing for each
options year. Other services provided shall be as follows:
Examinations: Radiology Service: General Diagnostic ultrasound shall
consist of abdominal studies (GB Biliary system, liver, spleen
pancreas, aorta, IVC, Renal). Petroperitoneal studies (Renal, Bladder,
and Aorta, IVC). Superficial Parts sonography (Breast, scrotum,
thyroid, prostate, popliteal and soft tissue). OB/GYN examinations.
Non-invasive vascular testing shall consist of abdominal doppler.
Cerbrovascular testing. Peripheral venous testing. Peripheral Arterial
Testing. Vascular Access Imaging. Cardiology Service: Echocardiography
shall consist of 2-D and M-Mode Echocardiography. Color Duplex
Echocardiography. Stress Echocardiography. In addition to the
performance of Ultrasound Procedures, Employees are to have knowledge
of the use of a personal computer with windows and micro soft word,
word processing and tape storage. Also they shall maintain all record
keeping for ultrasound related services for statistical analysis,
patient tracking, quality assurance and management review. In
performing these functions they will adhere to the patient privacy act.
Assume responsibility for notification to the designated liason to
stock and order ultrasound related supplies. Performance of quality
assurance program to include monitoring of equipment, technical and
service related issues, and diagnostic interpretation of examinations.
To be carried out by established guidelines. Provide prompt courteous
and professional services with respect to all patients, as well as any
interactions with the clinic staff and management. Commitment to
effective communication with the staff and management to ensure
complete satisfaction with ultrasound services provided. This will
include attendance to any meetings held specific to the provision of
ultrasound services to improve or correct the operation of said
services. Ensure full responsibility to promote continued educational
growth and implement any necessary changes in examination protocols,
techniques and diagnostic criteria to remain fully competent in the
performance of the services. The staff will act fully within the
professional conduct set forth by the nationally recognized society of
diagnostic medical sonograhers. At any time under the discretion of
the management of the VA outpatient clinic, any personnel not in
compliance with these guidelines or those of the clinic, will be
exempted from providing coverage at the facility, wit a two week notice
to ensure continuation of coverage. Examination schedule working hours
are negotiable but limited to Monday through Friday 8:00 a.m. to 4: 30
p.m., the current schedule is as follows: Radiology service: General
diagnostic examination and non invasive vascular ultrasound. Full eight
hour days (8:00 am to 4:30 p.m.) Tuesday and Thursday, Ultrasound room.
Excluding all federal holidays. Cardiology service: echocardiography
and same day vascular ultrasound. Full eight hour days. Tuesday and
Thursday. The current anticipated schedule is two three days per week,
however this may be modified depending on patient load. Invoices shall
be submitted monthly. The purchase order number and contract number
must appear on each invoice. Charges for echocardiography examinations
are separate from regular ultrasound exams. This is a "best value"
acquisition which shall be determined by a panel. Potential offerors
shall be rated and scored based on the following criteria: Past
performance in this exact type of work (Provide a minimum of three
references for ultrasound and echocardiogram per diem services
including names and phone numbers of responsible individuals who would
have first hand knowledge of performance offerors may provide past
problems encountered and corrective action). Qualification performing
this exact type of work (Include the qualifications of all personnel
that will perform services on this contract. Give number of years
experience and registered status, i.e. RDMS, RVT, RDCS, etc. and
compliance with current standards of continuing education must be
provided. Include documentation of experience with the accreditation
process for non-invasive vascular and general diagnostic ultrasound and
echocardiogram and provide all required administrative function fo the
acquisition of accreditation). Price (offeror is to provide cost for
services for contract award year and each option year). For any
