H -- LABORATORY TESTING SERVICE SOL NHLBI-PS-00-535 DUE 122899 POC Tabitha Davol, Purchasing agent, (301) 435-0368 Fax# (301) 480-3345 It is the intent of the National Heart, Lung, and Blood Institute to purchase the following Laboratory Testing Services : Bone Marrow Cytogenetics Tests, Two Hundred (200) each, Catalog No. 565, Peripheral Blood CytogeneticsTests, Ten (10) Each, Catalog No. 924, Chronic Lymphoproliferative (CLL) Disorders Cytogenetics, Tests, Ten (10) Each, Catalog No. 1914, Florescein in Hybridization(FISH) Tests,Twenty (20) Each, Catalog No. 4455. The period of performance is one year from the date of award. Scope of Work. The contractor must provide a) Bone Marrow Cytogenetics, b) Peripheral Blood Cytogenitics, c) CLL or Chronic Lymphproliferative disorder cytogenetics, d) Florescent in-situ Hybridization (FISH). Samples will be either bone marrow or pheriheral Blood. Samples will be picked up until 7P.M. of the day they are drawn and testing of sample will begin no later than 10:00 a.m. the following day. Emergency pickup must also be available at any time. Technical Criteria: Samples that contain sufficient cells,at least 20 metaphases must be examined and any cytogenetic abnormalities must be tabulated. Work must be performed in a CLIA certified lab. Contractor Tasks: The contractor will test samples for chromosome abnormalities to include complete karotyping including philadelphia Chromosome. All work will be done at the contractors facility using only contractor's equipment and supplies. Reports and Deliverables: Results will be reported to the appropriate attending physician for approval within 12 days of receipt of samples. Review and Approval: All work will meet industry and manufacturers standards and will be approved by the assigned attending physician. Government Furnished Property: The Government is not responsible to furnish anything in regards to this contract, except samples. The Standard Industrial Classification (SIC) Code 8734, Size Standard is $5.0 and will be used for this requirement. There are no set aside restrictions for this acquisition. Quotations are due 15 calendar days from the publication date of this synopsis. The quotation shall include a technical proposal outlining how the contractor proposes to complete the scope of work, a commercial price list with applicable government discount,courier service charges, prompt payment discount terms, business size and references from a minimum of 3 previous contractor's including contact name, address, and telephone numbers. Selection Criteria: Quotations will be evaluated on conformance to the government's scope of work, price, quality, and a proven record of successful past performance. Quotations are due on December 27, 1999 or (15) calendar days after the publication date of this synopsis whichever is later.
