This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in FAR subpart 12.6 and 13.5 and as supplemented with additional information included in this notice. Simplified acquisition procedures apply and the VA will conduct the procurement according to a combination of Parts 12 and 13. Solicitations are being requested under Public Law 104-262, The Veterans Health Care Eligibility Reform Act of 1996 and Title 38 United States Code Section 8153, with a Commercial Items format of terms, conditions, and provisions/clauses. This CBD announcement constitutes the only solicitation issued. A separate written solicitation will not be issued. Solicitation number Request for Quote (RFQ) 623-04-00 is assigned and shall be referenced on any proposals. Solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-14 and VAAR 97-7. The Standard Industrial Classification Code is 8011 and the small business size standard is 5 million dollars. The VA Medical Center, 1011 Honor Heights Drive, Muskogee, OK has a requirement for reading and interpretation of echocardiography tests as follows: This service will be required for an estimated 108 patients per month. The VA will deliver tests if contractor's location is in Muskogee, OK. Otherwise, contractor will be responsible for the pickup of tests. A detailed and complete reading and interpretation of the ECHO test will be accomplished for 2D, Doppler; and Color Flow. Contractor will notify referring physician immediately by telephone of patients' conditions that require urgent/emergent treatment. Upon availability from VA, interpretations and dictation will be done within 1 workday for inpatients and 3 workdays for outpatients. Contractor will dictate interpretations into VA transcriptions system. The VA will provide transcription services for dictation of tests. Billing will be made once a month, in arrears, for services rendered. Payment will be made for all tests readand interpreted in accordance with the requirements of this contract. The term of this contract will be for six months. The Contractor shall be licensed in a State, Territory, or Commonwealth of the United States or the District of Columbia. The Contractor shall be credentialed and privileged in accordance with VA regulations set forth in VA Circular 10-91-070, Credentialing and Privileging of Physicians and Dentists. The Contract Physician must be currently Board Certified in Internal Medicine and Cardiovascular Disease and maintain those certifications during the duration of the contract. No substitution of personnel shall be made unless prior approval is obtained from the Contracting Officer 60 days in advance. Personnel under this contract shall not be considered VA employees for any purpose and shall be considered employees of the Contractor. Workdays are considered 8am 4:30pm, Monday Friday, except federal holidays. Award will be made to the offeror determined to be the "best value" to the Government. Quotations shall be submitted on an "all or none basis". Partial Quotations shall be rejected. All potential offerors shall be available to perform the requirements of this contract immediately.
