This is a combined synopsis/solicitation for commercial items prepared according to the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation to be issued. The government intends to award a sole source contract to GE Capital Modular Space for the lease of a 9800 SF transition medical clinic, all offers submitted will be considered. Offers are being requested and no other written solicitation will be issued. Responses should reference Request for Proposal No. DADA10-00-R-0008. This synopsis/solicitation document incorporates the provisions and clauses that are in effect through the Federal Acquisition Circular 97-02. This procurement is unlisted. This recommended Standard Industrial Classification Code (SIC) for this procurement is 6512 with a corresponding size standard of $5 million. Firms responding should indicate whether or not they are small business, small woman owned business, small disadvantaged business,or 8(a) type business, according to the referenced SIC Code. The contract will be a firm-fixed price non-personal services contract. Contractor is to provide for lease for 1 year with two one-year options, with option to buy, a 9800 SF clinical building to be delivered and installed at Fort Hood, Texas not later than 10 Jan 2000. Building will consist of 53 rooms including: waiting area, glass-enclosed reception area, exam rooms, doctors offices, restrooms, X-ray room, darkroom for film developing, breakroom, laboratory, and administrative offices. Building will include HVAC system, all exam rooms will have sinks and cabinets, laboratory will have cabinets, refrigerator. X-ray room will be lead lined, darkroom will have double sink and " hose bib. Building will be asbestos free. Offeror will identify procedures for obtaining services for building maintenance and repair. Offeror will provide a minimum of three references for past performance. Building should be ready for occupancy on or about 10 Jan 2000.
