The VA Medical Center, 1011 Honor Heights Drive, Muskogee, OK is issuing a Request for Quote (RFQ), 623-07-00, Physician Primary Care Coverage. The Contractor will provide full-time primary care physician coverage for an "unscheduled clinic" at the Medical Center in Muskogee, OK and at the Outpatient Clinic in Tulsa, OK. Contractor will also provide physician coverage on an as-needed basis at both facilities. This procurement is for commercial services and is being conducted under Federal Acquisition Regulation (FAR) Subpart 13.5 test program, simplified acquisition procedures apply, and conducted with a combination of FAR Parts 12 and 15. The Standard Industrial Code is 8011 and the business size standard in $5 million. This is 100% Set-Aside for Small Business. A written solicitation will be issued. The RFQ will be issued on or about 1/4/00 and are due back on or about 2/4/00. The contract will be for a base year with an option to extend for one additional year. All responsible sources may submit an offer. Technical information will be required as part of the RFQ. Award will be made to the offeror determined to be the "best value" to the Government.
