DISCLOSURE SOFTWARE ENHANCEMENT AND SUPPORT The U.S. House of
Representatives (House) is seeking information on the enhancement and
post-implementation support of custom-developed financial disclosure
software that is posted to the House Intranet for downloading by House
Offices to use in filling out financial disclosure forms required by
the Ethics in Government Act. The goal is to continue to provide a
software system that's easy to use and requires a minimum of resources
to support. Application Description The current software has been
developed using Microsoft Visual Basic Ver. 5.0 with Sheridan Active
Threed Plus, Sheridan Designer Widgets, Apex Truedbgrid controls. The
software uses a Microsoft Access Database made up of ten tables one for
each schedule and one for general information. The software is
distributed via download from a web site or on CD-ROM. The user
performs installation. Microsoft Visual Basic Application Setup Wizard
generates the setup program. The software runs as a stand-alone
application in a Windows 32-bit environment (95, 98, NT). The reports
are created using Seagate Crystal Reports 6.0. Tables in the Access
Database: Tablename Fields General Information: name, address, phone,
status, district, state, employing office, termination date. Schedule
I -- Earned Income: Source, type, amount, Schedule II -- Payments in
Lieu of Honoraria and Charity: Source, activity, activity date, amount
Schedule III -- Assets and Unearned Income: Source, asset, type,
amount, value, transaction Schedule IV -- Transactions: Source, asset,
transaction type, transaction date, amount Schedule V -- Liabilities:
Source, creditor, type, amount Schedule VI -- Gifts: Source,
description, value Schedule VII -- Travel Payments and Reimbursements.:
Source, dates, Point of Departure/Destination/Point of Return, lodging,
food, family member included, any time not at sponsor's expense
Schedule VIII -- Positions: Position, organization Schedule IX --
Agreements: Agreement date, parties, terms of agreement In responding
to this survey, the software solution provider should include all of
the features of the current software with the addition/modification of
the following features: Ability to add, alter, or delete fields of
information, as needed. Ability to export data to an external MS Access
database. Modifications to existing database structure (if required).
The tasks to be completed by the contractor shall include: Gathering
User Requirements (modifications requested) Making the changes to the
application (see above) Updating the On-line Help files to correlate to
the modified application Creating the (self extracting) Installation
program Burning approximately 40 CD's (user-supplied) containing the
Installation program for organizations without intranet access Creating
the Multiple-User version of the application Creating the installation
program for the Multi-User version Instructions The House is
requesting vendors to providethe following information based on the

Contact, and Email Address. 2. Manufacturer(s)/Publisher(s) of the
proposed software solution, purchase price, and annual maintenance
costs 3. Proposed technical architecture and required hardware
specifications, (brand name products preferred) and estimated hardware
prices 4. Estimated Implementation Timeline and costs 5. Estimated
training costs 6. Estimated costs of help-desk support 7. Estimated
costs of on-site support/service for one year 8. Estimated cost of
creating/supplying the CDs All responses to this RFI should be
