The Aberdeen Area Indian Health Service proposes to award a Firm Fixed-Price Contract for a Contractor to provide delivery of on-site equipment and technical personnel to support cataract surgery at the Rosebud PHS, IHS Hosptial, Soldier Creek Road, Rosebud, South Dakota 57570, Attn: Optometry Department. Expected surgical load will be approximately eight (8) surgical cases per day, one (1) day per month for twelve (12) months. The following equipment to be furnished by the contractor and to be leased by the Rosebud Service Unit are as follows: (a) Two phako units (primary and backup).; (b) Operating Microscope; (c) Cautery System; (d) Lens implants (IOL's, primary and back-up for each case; (e) Custom surgical packs (gowns, gloves, drapes, syringes, etc.); (f) Surgical Instruments (cataract-specific); (g) Vitrectomy packs; (h) Operating stool with wrist pads; (i) Certified technical support to set up, maintain during surgery and remove all equipment at the end of the surgical day (one technician is sufficient); (j) All needed handpieces, disposable items (tubing, hoses, etc.) as required for the equipment to be used; (k) YAG laser. The Rosebud Service Unit will provide the Board Certified Ophthalmologist, the operating room facility/staff, the pre-operative work-up (H&P, CBC, ECG, Biometry, laboratory work, etc, as deemed necessary) and the Anesthetist. The contract will be responsible for the transportation, maintenance, upkeep and upgrading of all equipment and for all consequences arising due to neglegience in the upgrading and upkeep of the equipment.
