synopsis/solicitation for commercial items prepared in accordance with
the subparts in Federal Acquisition Regulation (FAR) 12.6 and Veterans
Administration Acquisition Regulations (VAAR) as supplemented with
additional information included in this notice. This announcement
constitutes the only solicitation; quotations are being requested and
a written solicitation will not be issued. Solicitation number
673-47-00 applies and is issued as a request for quotation. This
solicitation is a Total Small Business Set-aside in accordance with FAR
clause 52.219-6. This solicitation is being conducted using the test
program for certain commercial items in accordance with FAR Subpart
13-5; simplified acquisition procedures apply; and this procurement
will be conducted according to a combination of FAR Parts 12
(Acquisition of Commercial Items) and 13 (Simplified Acquisition
Procedures). This solicitation document and incorporated provisions and
clauses are those in effect through Federal Acquisition Circular 97-15.
The Standard Industrial Classification Code is 8099 and the business
size standard is $5 million. PRICING SHALL BE SUBMITTED FOR 1.
Diagnostic radiological technology service estimated 2,880 hr per
6-month period In Accordance With (IAW) Statement of Work. STATEMENT OF
WORK: The contractor shall provide temporary staffing of diagnostic
radiological technologist for James A. Haley Veterans' Hospital, 13000
Bruce B. Downs Blvd., Tampa, Florida 33612, for the period of six
calendar months from date of award, with an option to extend an
additional 6 month period. The requirement is for three (3) full time
radiological technologists. The contractor shall perform a variety of
routine radiographic examinations in the out patient center;
intravenous pylograms (IVP's); and difficult portable radiographic
examinations. Each technologist will follow established radiation safe
guideline, will explain to the patient the examination and what is
involved, selects and sets technical factors. Adjusts accessory
equipment required, and makes exposures necessary for the requested
procedure. Knowledge of anatomy and physiology such as location,
appearance and function of the various major systems including the
muscular, circulatory, lymphatic, respiratory, digestive, and urinary
systems in order to interpret the examination request accurately; to
understand the functioning and interrelationship of the various organs
appearing on the film and various stages of the examination to judge
the acceptability of radiograph for diagnostic use. Knowledge of
radiographic exposure techniques including use of formulae for
determining technical factors, how to control density, contrast, and
distortion, and use of intensifying screens, buckys, collimators,
filters, grids, and other accessories to produce radiographs of high
diagnostic quality with a minimum of retakes. Knowledge and skill in
positioning of patients for a wide variety of difficult radiographic
examinations. The work involves independent performance of a varietyof
routine radiographic examinations. The technicians occasionally must
use basic formulae to calculate technical factors, for example, to
accommodate for patients who are substantially more robust or frail
than normal. When assessing or providing patient care the technicians
will consider the age of the patient as it relates to ability to
understand directions, provide objective self assessment or any other
medical milieu appropriate to the overall treatment plan.
QUALIFICATIONS: Personnel must have current ARRT certification. TIME OF
PERFORMANCE: Monday thru Friday 8:00am to 4:30pm, except Federal
Holidays. Federal holidays observed are Martin Luther King Day,
Presidents Day, Memorial Day, Independence Day, Labor Day, Columbus
Day, Veterans Day, Thanksgiving Day, Christmas Day, New Year's Day, for
10 total. Contractor shall provide radiation monitoring badges. VA will
provide all supplies and equipment except for radiation monitoring
badges required for the performance of this contract. It is the
intentionof the VA Hospital to provide training in the current policies
and procedures at this facility. Therefore, continuity of personnel
assigned to perform this contract is desirable. Personnel performing
the above program must be assigned for a minimum of 3 months
consecutively. While it is desirable to have personnel for the complete
period of contract, it is acceptable as an option for a minimum 3
consecutive months time. VAAR clause 852.237-7, Indemnification and
Medical Liability Insurance is applicable to this solicitation and
contract. The contractor shall keep records and withhold appropriate
Federal and State taxes and fees (FICA. SS, Income Tax, etc.). The
contractor shall ensure proper that all employees receive mandatory
training as required by OSHA, JCAHO, etc. The contractor shall ensure
proper dress (smock, shirt, pants, shoes, etc.) and conduct of
employees. Such personnel shall not be considered VA employees for any
purpose. Contractor will submit monthly, in arrears, invoices covering
services rendered during the month to Chief, Fiscal Service (04), James
A. Haley Veterans Hospital, 13000 Bruce B. Downs Blvd., Tampa, FL
33612. Quotations will be considered only from bidders who are
regularly established in the business called for and who financially
responsible and have the necessary personnel to furnish service in the
volume required for all the items under this contract. Successful
bidder shall meet all requirements of Federal, State, County, or City
codes regarding operations of this type of service. The government may
make multiple awards to the responsible offerors whose offers are
advantageous to the government in accordance with FAR clause 52.216-27.
