is a combined synopsis/solicitation for commercial items prepared in
accordance with the Federal Acquisition Regulation (FAR) sub part
12.6., as supplemented with additional information included in this
notice. This announcement constitutes the solicitation; proposals are
being requested and a written solicitation is available. Solicitation
number RFP-797-FDF1-00-0035 is issued as a Request for Proposal. The
solicitation document and incorporated provisions and clauses are those
in effect through FAC 97-15. The SIC code is 3845 and the small
business size standard is 500 employees. The solicitation is issued as
an IDIQ for a basic period of one year and 4 annual options to extend
the term of the contract. The contractor shall provide at a minimum
during the basic year five systems with the following salient
characteristics: lightweight, totally enclosed, portable diagnostic
ultrasound unit, including dual frequency probes capable of obtaining
fine detail of superficial structures or increased depth penetration;
multiple interchangeable probes; 30 channels in order to receive echo
signals for better image resolution; PC based software; reliable, easy
to use, constructed to provide long life, usability, and provide for
low operating and maintenance costs; minimum of 256 gray shades; 4
focal zones. Procured equipment must be approved for use under the WRM
patient treatment readiness assemblage list. Delivery orders will be
placed against the resultant contract. Delivery shall be FOB
Destination within CONUS. Delivery is desired within 30 days ARO. This
is a total small business setaside. Descriptive literature shall be
furnished as part of the proposal along with a list of support items
and parts list. Offerors are required to provide a standard price list
and the price offered to the government. Offerors responding to this
announcement shall provide the information contained in FAR clause
52.212-1, Instructions to Offerors Commercial Items and shall include
a completed copy of provision 52.212-3, Offeror Representations and
Certification -- Commercial Items. The Offeror shall also provide its
Commercial and Government Entity (CAGE) code, Contractor Establishment
Code (DUNS number) and Tax Identification nuber. Clause 52.212-2,
Evaluation -- Commerical Items, applies to this acquisition. The
Government will award a contract resulting from this
synopsis/solicitation to the responsible offeror whose offer conforming
to this synopsis/solicitation will be most advantageous to the
Government, price and other factors considered. (1) Technical
capability, (2) past performance, and (3) price. Technical and past
performance when combined are significantly more important than price.
Offerors shall provide technical information and a minimum of three
past performance references, preferably government references. The
resulting award will incorporate the requirements of the clause at FAR
52.212-4, Contract Terms and Conditions -- Commercial Items. Clause
52.212-5, Contract Terms and Conditions Required to Implement Statutes
or Executive Orders -- Commercial Items, applies to this acquisition.
