combined synopsis/solicitation for commercial items prepared in
accordance with the format in subpart 12.6 as supplemented with
additional information included in this notice. This announcement
constitutes the only solicitation; proposals are being requested and a
written solicitation will not be issued. This solicitation, No
F02604-00-Q0091 is issued as a Request for Quotation (RFQ). Funds for
this acquisition are available. This solicitation document and
incorporated provisions and clauses are those in effect through Federal
Acquisition Circular 97-15. This acquisition is 100% set-aside for
small business SIC: 1781 Size Std: $7.0. Contract Line Item(s): (CLIN)
0001; 1. SCOPE: The project covered by this statement of work consists
in furnishing all plant, labor, equipment, appliances, and materials
and performing all operations in connection with the complete repair of
water well #11. All work will be in strict accordance with the terms
and provisions of the contract. 2. REMOVAL AND REINSTALLATION OF A 250
HP MOTOR, SHAFT, AND BOWL ASSEMBLY TYPE 12 LKH: Contractor shall
remove all well column assemblies and associated piping as necessary to
determine the equipment malfunctions and facilitate the repair of well
#11. All equipment and materials shall be neatly stacked on the ground
at the job site. Suitable cover shall be provided to protect shaft
threads. Disconnected pipes and well casing shall be capped to prevent
contamination to the well site. The contractor shall reinstall the
turbine pump and all other removed appurtenances after completion of
specified work. 3. STERILIZATION: Disinfecting of the well by
chlorinating will be accomplished by Government personnel after
completion of the project. 4. WATER WELL PUMP WORK TO BE PERFORMED BY
THE CONTRACTOR 4.1 Remove and reinstall oil-lubricated well, turbine
pump, motor, and appurtenances in 18" diameter well. Bowl setting is at
590-foot level. 4.2 Contractor shall inspect the well components to
determine the cause and extent of the malfunction.Upon completion of
inspection, the contractor shall submit a report of findings and
recommendations for repair to the Contracting Officer. Upon completion
of review, the Contracting Officer will instruct the contractor to
proceed. 4.3 REPLACEMENT PARTS: The contractor shall furnish and
install all parts authorized by the Contracting Officer as required for
repair of the pump column. 5. TESTS: After the installation is
complete, and at such time as the Contracting Officer may direct, the
contractor shall conduct an operation test for approval. The equipment
shall be demonstrated to operate in accordance with the requirements
of this specification. The test shall be performed in the presence of
the Contracting Officer or their authorized representative. The
contractor shall furnish all instruments and personnel required for the
test. 6. SUBMITTALS: The contractor shall provide copies of pump curves
and product data on all replaced parts. 7. WARRANTY: The contractor
shall provide a 1-year warranty on all work and material. (CLIN 0002)

repair of water well #14. All work will be in strict accordance with
the terms and provisions of the contract. 2. REMOVE PUMP AND ALL PIPE,
T.V., WELL CASING, AND REPAIR/REPLACE WELL COMPONENTS AS NECESSARY:
Contractor shall remove all well pipe and components as necessary to
allow for a television camera to be inserted into the existing well
casing to determine the cause of well malfunction and to facilitate
repair. All equipment and materials shall be neatly stacked on the
ground at the job site. Suitable cover shall be provided to protect
pipe threads. The well will be capped when work is halted for any
extended period of time to prevent contamination. The contractor shall
repair or replace all pump components and appurtenances after
completion of specified work to ensure properwell operation. 3.
STERILIZATION: Disinfecting of the well by chlorinating will be
accomplished by Government personnel after completion of the project.
4. WATER WELL WORK TO BE PERFORMED BY THE CONTRACTOR 4.1 Remove the
well pump and pipe to allow for the inspection of the well's casing
using a television camera. Repair or replace a 125 hp, 460 volt, 1770
rpm pump and motor and associated components as required. Pump is set
at 472 feet. 4.2 Contractor shall inspect the well casing using a
television camera. Contractor will also inspect all other well
components to determine the cause and extent of the well's malfunction.
Upon completion of inspection, the contractor shall submit a report of
findings and recommendations for repair to the Contracting Officer.
Upon completion of review, the Contracting Officer will instruct the
contractor to proceed. 4.3 The contractor shall furnish and install all
parts authorized by the Contracting Officer as required for repair of

parts. 7. WARRANTY: The contractor shall provide a 1-year warranty on
all work and material. This is a brand name or equal requirement the
FAR Clause 52.211-6 applies. Basis of award: award will be made to the
lowest-price Offeror, best demonstrating their ability to satisfy the
technical capabilities of the items specified. Offerors of any equal
must provide descriptive literature to this office for evaluation.
Required delivery: 30 days ARO. FOB: Destination to Luke AFB, AZ. All
prices shall include all Federal, State and Local taxes. The following
