The work will include but not be limited to standardize and increase number of private rooms, increase the size or rooms such as the Anesthesia Evaluation and P.A.T., provide offices for employees currently located elsewhere in the hospital, and other functions such as EKG and Blood Draw which are presently located on other floors of the hospital, provide a space for a patient reception and waiting area and a conference room for the patient and/or family, increase number of patient rooms and toilet rooms and provide ample storage to accommodate equipment such as B.P. equipment, monitors, EKG equipment, SICU hospital chairs, crash cart and other rolling standing pieces of equipment at the Department of Veterans Affairs Chicago Healthcare System, West Side Division, 820 South Damen Avenue, Chicago, IL 60612. Estimated construction cost is between $2,000,000 and $5,000,000.00. The area of consideration is restricted to 100 miles of the VA Chicago Healthcare System, West Side Division, Chicago, IL. Selection criteria will be based on(1) Past performance on similar projects, maintaining project schedules and project budgets; (2) Overall design experience and capability with architectural and mechanical projects; (3) Qualifications of staff to be assigned, with special emphasis on field personnel to be utilized; (4) Cost control effectiveness; and (5) Ability of A/E to produce specifications on a 3.5 disk for microsoft Word format release 7.0., ability to produce drawings on a CAD system fully compatible with Auto CAD Release 13 format on 3.5 disk.
