V -- TRANSPORTATION PACKING AND DELIVERING SERVICES FOR NEW POPC SOL RFP672-237-00 DUE 021800 POC Rey F Clemente (787) 641-2955 The San Juan VA Medical Center, San Juan, Puerto Rico has a requirement for a service contract for Transporting, Packing and Delivering Services from the Old Ponce Outpatient Clinic, (POPC), Ponce Outpatient Clinic facilities located at Urb. Industrial Reparada, Calle Principal Lote #1, Ponce, Puerto Rico to the New Ponce Outpatient Clinic located at Santiago De Los Caballeros Avenue, Finca Multedo Estrella Lote #37, State Road PR 14 Km 3.3 (Malecon Avenue), Ponce, Puerto Rico. This solicitation is set aside for Small Business. SIC Code is 4783. The period of performance will begin approximately on or about February 28, 2000. VA anticipates award of a firm fixed price contract based on the contents of this solicitation. The Contractor shall provide all labor and materials to pack, transport & deliver equipment, accessories & records including supervision from the existing POPC to the new POPC. The scope of work will include: 1. The labor and material necessary to pack all identified items in boxes. 2. Identification of boxes with room numbers as per items identified. (Identifying the room # at the new facility). 3. Transportation of all boxes from existing building to new facilities. 4. Delivery of boxes to the rooms as per identification. 5. Records will be packed as per alphabetical or numerical order and will be organized in new and old file bins in the same organized order. The date to start this movement will be on or about Feb 28, 2000 or as requested by the COTR assigned to this project. The Contractor shall be responsible for requirements in accordance with all terms, conditions, provisions, specifications, and schedules of this solicitation herein. The Government is not obligated to any specified amount of products and/or services, but will be obligated to provide payments for all items/services requested in accordance with this solicitation.
